The chastity belt was actually used, it's set dressing in the TF2 adult swim commercial and can be seen right off the bat sitting on the TV. Chains in the same position as it is in the commercial. Leads me to believe it was more just set dressing than anything that was planned as a cosmetic.

Item specifically, it's also possible that the Molotov's special belt could have also been a cut item that could've potentially been obtained through Poker Night at the Inventory 2, which was a gambling game featuring Brock Samson himself (someone that has close ties to Molotov) and already had numerous rewards you could've obtained through special bounties or completing certain requirements within the gambling simulation, such as Brock's Locks, an item you could have gotten in Tf2 through these means that's Brock's Wig. Perhaps this Chastity Belt, given its spicy nature and the spicy nature of gambling as a whole, could've been what one of the original rewards was planned to be.

You can see it right at the start of the commercial sitting on the tv, with the chains in the leak model in the exact same position as they lay on the tv.

About the Mario stuff, at the time it was perfectly legal in Japan for that sort of thing, it was considered "comedic" and some times simply irrelevant. You could find that sort of thing in manga and anime and some videogames, all directed to children. The reason they removed it is most likely to prevent the game from being banned in other countries where there were already laws to prevent that type of stuff in digital media (Japan got the later) directed to children.

Slight note about DangenRanpa V3. Because the series was already a success by this point, the publisher never rejected the game causing retooling. It's core concept is grim but had already been the premise of many successful games and spinoffs by this point. The incident you're thinking of actually happened to the first game, which was released back in 2010. That being said, V3 was a major source of controversy in South Korea, as a teenage girl who murdered someone was found to have been a fan of online roleplaying; and a series of misunderstandings later many news outlets incorrectly claimed she committed the murder because of her fascination with DangenRanpa RP. Because of this the game was banned in Korea.
