You don't change the locks before they leave, you do that after they let the door hit their butts on the way out. Also put cameras around the home to deal with them.

so the grandparents show up for his birthday but don’t even question where the rest of the family is? And they don’t pitch in with groceries or anything? Everyone knew he’d be left to fend for himself, and somehow he didn’t notice the state of his room until the ownership papers

So... When the Grandparents show up for his birthday, none of them even ask where the family is? They don't help getting groceries into the fridge of the kid they care so much about? Everybody but OP knew he would be alone and have to care for himself? And he never noticed how much of a mess his own room was, until he got the paperwork of ownership? Sounds legit.
