My hospital says I can do diet or shakes, I've chosen shakes as with food, it's too easy to have and extra mouthful here and there. Well, that's how I got to needing the surgery in the first place. Another 8 days to go.

I so appreciate the video representing the damage that can be done to our livers. (GSV in two weeks) It's scary as it should be because this is totally controllable by our choices Preop. I'm in a three week diet. Ive lost some pounds already but haven't stuck to it 100%, now.... I think I'll go back to. 100%. So two weeks can help avoid this. Id hate to do this to not only myself but my surgeon and care team.

I'm in Australia and saw my surgeon yesterday, decided on self funded gastric bypass in 7 days. I'm 70 and had a lapband 15 years ago. This will be removed. I was told that i didn't need to do anything preoperatively. The anaesthetist told me I'll be in high dependency unit for 1 to 2 nights due to severe sleep apnea, then on the ward another 2 days. I'll have 2 tubes, a catheter and a neck iv. On the 4th day they do a test to see that everything is flowing properly, then get discharged. Having done keto diets on and off throughout the years i knew that fluids only, post operatively, would put me into ketosis. So i thought about getting onto that before hand to get over the keto flu prior to surgery. After watching your video however the prime goal now is liver shrinkage. I'll do the intense protein shake diet with a vegetable chicken soup as a meal each night, then fast on the 6th day. I want as easy a transition as possible and a good field for the doc. I'm surprised he didn't tell me to do this though and he was very unfazed, when i asked if i had to do anything preoperatively. Obviously with a 4 day stay and intensive care plus all the tubes, he's expecting issues. I'll just not give it a chance. Thanks for the very clear explanation and visuals. I'm on it as of today to be ready for next week. 🤞

@@DoctorJP yes you are correct. I've spent 4 nights in critical care although I only needed 2 days of that side of the service. I had a central line. 2 tubes, a catheter and apparently this is common practice with my doctor. I was on iv antibiotics, potassium and heaps of fluids. I'm also to give shots of Clexane for the next 20 days. The surgeon said that the lapband had caused adhesions on my liver which took an extra 1.5 hours to cut away. I also had a bile leak from the liver (no gallbladder). He did say my liver shrinkage diet gave me a small clean liver except it was adhered. Apart from some gas bubbles I feel pretty good. I'm home now and on free fluids which includes yoghurt, creamy soup and obviously broth and jelly. In the hospital they even gave me ice cream. I couldn't believe it. I can drink tea and decaf with milk as well. All my prep for clear fluids is semi wasted ha ha.

Dr. Pilcher thank you for showing that video, it's very helpful to me during my research as I'm considering bariatric surgery. It would be nice if there were a surgical tool that perhaps expands (think wings) to create a flat surface for the liver to rest on to minimize damage to the liver. That split looked really bad.

I like the way you think, Natalie, and back in the old days when we made a big huge cut up and down the abdomen we did have a larger "dam" shaped retractor that was less likely to cut into the liver. These days with little incisions we can only put small things in. On balance it's worth it because the best majority of people lose weight well and do not have any liver damage.

I know at our support sessions when we had it, many gastric bypass patients have stated drinking water "hurts" them going down the esophagus. I have remedied that so I get my water but still don't understand why this is. Things that make you go hmmmmmm

Dr. Pilcher, thanks for making this informative video. I found it to be beneficial. I have the Laparoscopic Duodenal Switch with Single Anastomosis this Wednesday, December 22, at 11:30 AM. I was placed on the liquid diet a few days ago (earlier than expected) due to impending concerns from the new COVID variant. Nonetheless, I have been modifying my diet for surgery for weeks now and feel confident about liver shrinkage. I will take your recommendation on going more aggressive and think I will be fine. Question- is adding salt and pepper to my broth hurtful or helpful at this point?

Hi Dr Pilcher, is it safe to do Dr.Sebi's liver and gall bladder Cleanse prior to Gastric Bypass Also, is it ok to do a water only fast prior to Surgery.? The reason I ask is because when doing a water only fast, after at least 5 days a person is in Autophagy and I watched Heidi's video on your channel she mentioned that during surgery we are "pumed up with fluids" I don't know what the fluids are but if in Autophagy, its necessary to do a slow "Refeed" and if not done properly it could cause side effects like heart palpitations. So im wondering if doing an extended water only fast is safe prior to surgery. Thank you. Also, I want to "Thank You" from the whole world because you are "THE ONLY ONE" who responds to peoples questions on KZbin

Man! I remember my 2 week liquid diet plan. 3 protein shakes and up to 3 snacks. Broth, sf jello and sf Popsicles. Well I did it to a tee and had the sweats, Nausea, heart palpitations and starving. Well I was so excited that I did it. Well I went in for my weigh in. I lost only 3lbs! I couldn't believe it I cried. Then my PA asked what I was doing. I said I did everything to a letter. So I had to cut out any of the snacks just do the shakes. Day of surgery I get a call 2 hours before I was supposed to be there my surgeon was sick. Fast-forward 2 months. I have to do this again. But I thought Well the first round I did what they said I barely lost, so I am doing it my way. I did 5 protein shakes a day instead. And I lost 8lbs. I don't know exactly why that happened. Weird

Hi Jason, I'm awaiting my date for my gallbladder removal. My surgeon has suggested 1 of 2 diets. (One being mainly milk based with the odd few yoghurts/ Bovril or stock cube in hot water) and the other a very plain very small meal based one. I'm scared if I do the food one I'll be unsatisfied and want more, as I love food so much (just being honest, obviously that's why I'm here and needing removal!) So I'm opting for the milk based one even though I'm not a fan of drinking milk unless it's in tea or coffee. I'm very scared about doing this as my family depend on me a lot (I have small children and my husband is disabled) I'm scared about not having enough energy, feeling faint etc. Will drinking more fluids like water help this? Also scared about recovery time. Do you have any tips for me as I'm very anxious about the procedure. Thank you :)

You mentioned in a video about new information about the sleeve having some new information since i think 2021 or so. People gaining back weight and acid reflux. Is your last bypass vs. Sleeve still current? I told my surgeon i want to do the sleeve but in starting to have second thoughts. I'm leaning towards bybass now.

Sorry to be slow in replying, and I hope this answer is still pertinent, but in my opinion, Gaviscon is OK on the liver shrinking diet.

Thank you I knew there had to be a good reason other then testing if the pt. will be compliant . But now I know the why , what about being that over achiever , Example: start with 2x shakes and a meal a day . over achiever Start 1x and a meal a day

I had a question..I have been looking forward to having the gastric bypass but i had my consultation with my surgeon today and in the notes it said we will be doing the gastric bypass or sleeve. When I asked him he said if they can't do the bypass field to complications that they would do the sleeve instead. What does this mean? I would rather have the bypass but don't want to wake up after surgery and have the sleeve done.

I wish I could answer that for sure RUME, but I suppose your surgeon just wants to have all the options on the table. Now that you've had a chance to think about it, I think it would be fair to ask in what percentage of cases your surgeon plans a gastric bypass then makes an intra-operative decision for Sleeve? If you feel strongly, it should be an option for you to instruct the surgeon to stop the operation if they can't do a gastric bypass (rather than fall back to the Sleeve).

Thank you doctor. 2 questions. Is it safe to say that the bigger you are he bigger and fattier the liver? Or could it just be random. Also is is a guarantee that if you follow the pre-op diet your liver will be small and healthy. Thank you.

Thanks for your question Joan. There is a rough correlation between total body size and liver enlargement, but other factors have a strong influence such as body shape, medical conditions, and age. In your question, you used the word "guarantee." There really aren't any guarantees in medicine, sorry. And to be honest, I have seen one or two patients who seemed to do a very good job on their liver shrinkage diet but still in surgery the liver was too big for us to accomplish the surgery. Both those patients went back and dieted some more, then had successful surgery. Seems it would be smart for you to visit back with your surgery team on this. Best of luck on your journey!

I think i'm in trouble. I am on a Liver shrinking diet for gallbladder surgery. I've eaten under 100g of carbs a day (anywhere from 26 to 60) and keeping my calories under 1000. I'm on day 4 and I see a slight reduction in my belly but I am not in ketosis yet. Am I gonna be able to get surgery on May 14?

Ok, I do have a question, how would you know if it’s working? Should people with known NAFL and a higher BMI stay on the diet longer?

That's a smart question, but my advice is not to do the diet longer than your surgeon recommends. If anything, you may intensify the diet by getting closer to fasting in the last couple of days before surgery. Please be sure you are keeping up with your hydration during the liver shrinkage diet.

Hi doc. I have a vsg scheduled and they gave me a 2 week preop diet, if I were to do the 7 day one you were discussing my results should be about the same you think? I’m on day 4 and feel like I’m losing it lol.

I’m 8 months post op (not with this place) and it’s so hard to get three meals a day because of my lack of appetite. I force myself to do the 2 shakes and a meal a day. And a snack.

Thanks for asking Michael. We are actually pretty well practiced with bringing patients from other parts of the country for surgery. The COVID pandemic pushed us to get better at remote patient connections, so we are much more comfortable accomplishing the long term follow up as well. Hi doc, my surgery is in 4 days and unfortunately my surgeon has no requirement for a pre-op diet. He’s an India-based bariatric surgeon and seen this is quite common there. After watching this I think I should do something. Can I just do a water fast for the 4 days to surgery? would it do much to shrink my liver?
