@@lisarobb3064 not in the UK. Unfortunately the receptionists always push ear infections to the practise nurses/paramedic nowadays who don't always get treatment right. The nurse, in their defence would not have had access to the letter written to the GP. I have chronic health problems so am at my GPs a lot so I know the system and it needed the patient to push to be seen by the GP who had the letter.

I wish you were able to take a swab, send it to the lab and have the results sent to you and the GP! It would make so much more sense. With how knowledgeable you are I’m sure you are capable of taking a swab and sending it to the lab. Instead of 4 visits (or 5) it could have been 2!

I think the same thing every time he mentions a patient needing a swab! It seems that it would expedite treatment, and avoid nonsense like this patient being told they didn't have an infection. As far as I am concerned, that GP/nurse should have to pay the fee of this patient needing to return to Neel so soon. It's not right that through what seems a lazy oversight the patient has to deal with another fee that could have most likely been avoided. I will never understand why the medical systems there and over here insist on being so overly complicated.

The issue with that is still the same; it will create a contained vacuum with in the ear canal, and the weakest point in this enclosed space will be the tympanic membrane (eardrum). This, in turn, can relatively easily cause the eardrum to rupture. So, not a good idea.

Audiologists aren’t doctors or surgeons. In the UK they aren’t allowed to write prescriptions or make diagnoses, that’s why Neel talks about writing letters to the GP. He obviously can suggest what he thinks is wrong with the patient’s ear but it’s up to the GP to take swabs and make the actual diagnosis and then go on to prescribe the correct medication. Neel (and other audiologists) can prescribe over the counter medication but nothing more. I hope you don’t mind me correcting you, I don’t mean to be rude.

Neel, I have to question the GP not prescribing antibiotics when it is very evident an infection exists. Perhaps a better examination by the GP should have taken place. Well, hopefully the second time around will get a result from the GP, the poor patient can't carry on like this. As always, a great informative video Neel, a little more knowledge I've stored in my empty brain cells. At the rate I'm learning about these procedures I'll have a full house 😅. Please forgive my ignorance, but when you were trying to put the full-size tube in the ear, would it have been dangerous to turn the suction off so you could get the tube past the swelling and then turn it on? I used to get ear infections all the time as a kid, so I know that poor patient was in a lot of pain from that. Thanks for being so compassionate in your treatment! Incidentally, I'm guessing the pain and discomfort was why you didn't go in with a Jobson horn. I know I'd have a bad time if I went in and the doctor started treating my infected canals like a bucket of ice cream 🤮

Question! I don't know how the actual machine used for the suction actually works, but in the case of a collapsing ear canal entrance, is it not possible to stop the suction until you get through the collapsing part and then start it up again? Or does that create its own issues? Thanks!

I was sooo angry,I'm not afraid to admit it,how could he not see the infection. Luckily you were able to help AGAIN, all I can say is I was disgusted that he walked shotgun over you're diagnosis. Does your fine end suction probe work continuously, or does it function like a Yankauer? Outof ignorance, I would have expected the latter so worth asking... Meaning, can you break the vacuum immediately when the canal collapses, just by lifting a finger?

I still wonder about the irrigation if you want to avoid water because it works as incubation media. What if the water was threated so it itself would be antibacteria (combined with iodine, hydrogen peroxide)? The peroxide will dissipate quickly but the iodine for example won't I think so in the end it will be as aplicating local antiseptic.

I do find it annoying now that if you have an ear infection (I'm speaking about the UK) that you get palmed off to the practise paramedic or nurse. I had them trying to diagnose me over the phone, of course things didn't get better and on my third call I finally got to see a doctor. Now I am a regular at my doctor's practise as I have multiple health problems so I know to just keep pushing for help if something feels wrong but a generally fit and healthy person may just accept what the nurse says.
