Hey Adam, I'm in med school now and will be an MD next year. Thank you for being this honest and vulnerable. I personally learned a lot from learning your story and relating more to what patients suffer through sometimes. If it's any consolation, I plan to share this with some classmates and one day, your truth might help future patients going through this to find more empathy and understanding in their doctor.

@@InTheMoneyAdam I showed this video to some med students and nursing students. So often we see so many “common” psych patients that we can be way too quick to judge what’s going on with someone. While we might be right most of the time, the consequences of being wrong are enormous and can be devastating as you’ve illustrated. We all found this very helpful. Thanks for sharing.

It will definitely help. As a physician, until I went through my own case of "idiopathic" chronic illnesses, did I start understanding what my patients felt. Certainly don't rush rounds anymore. Sometimes, even if we don't know what the fuck is going on, simply giving a little bit of extra respect and attention goes a long way.

Hi 👋🏻 I’m an ICU nurse. Super suspicious that they haven’t been able to find a source of infection (and I hope they tested for Lyme disease) when you and your brother got sick at the same time. Since they have been unsuccessful in finding a source, It may be beneficial to consider getting genetic testing (again) to see if you and your brother have familial encephalitis. Lastly, there are many types of autoimmune encephalitis (ADEM, Anti-NMDAR receptor encephalitis, hashimoto’s encephalopathy, LG11/CASPR2-antibody encephalitis, limbic encephalitis, Rasmussen’s encephalitis, etc). I hope you have a Rheumatologist on your team as well. They are the autoimmune experts and autoimmune conditions love to “play together”.

ED Physician Here. First off - thanks for sharing your story. Watched the whole thing and thats about 50:00 minutes longer than I spend with most patients. What you describe sounds like Anti-NMDA receptor (NMDAR) encephalitis 100%. I remember you mentioning your antibodies coming back negative during the saga, but there are a huge list of antibiodies that can contribute, most of which wouldn't be on a typical spinal fluid panel unless you had an astute doc looking out for them/someone assisting with what tests to order. Seems like its on the mend now. Know that if it is NMDAR Encephalitis it can take a year or two to recover. Immunosuppressive therapy (steroids) treatment responsiveness is a hallmark of NMDAR/Autoimmune Encephalitis. I think you nailed the diagnosis long ago. I'm sorry that it took so long for someone to hear your story. I had a case or two of this in residency so it stays fresh in my mind. Thanks for the reminder. And hey thanks for teaching me about covered calls. I wish I would have found your stuff before I found WSBs...the ole P/L would be looking a lot better. Praying for you man. There is a book by a guy by the name of Richard Swanson called Contentment. Quick read. He talks about not wasting the pain. You paid a big price to be able to put that video up today and to have the perspective that you have now. Hoping the roids get this thing licked for you. - Reese

Does this mean the neurons in his brain were having a hard time increasing synaptic strength since NMDA receptors are necessary for LTP? He also said something about myelin sheath degeneration so one way or another it sounds like his neurons were having a hard time of communicating with one another.
@reesedare22082 жыл бұрын
@@kurtislyons8366 Yo, honestly there is so much crossover between the two that its very hard to tell the difference. Really its just keeping your differnential wider than the common presentations of the psych stuff. But - Viral Prodrome + Neuro symptoms + Psych in a previously psychiatrically normal (assuming) patient. Worth considering. They'll have normal MRI and LP results off the bat. The NMDA Receptor antibody test is usually a send out so you have to have a neuro team willing to treat for a few days or a week even on this assumed dx. The case that sticks out in my mind was a 16 year old straight A student who was normal per her parents but for the past week had increasingly odd behavior after a recent viral illness. Emotionally labile, aggressive/hypersexual which per the parents was not her norm at all. After we did a psych panel, another doc a lot older and smart than me suggested we do the tap/send IgG. Sure enough, when i chart checked her 3-4 days later she was positive. I think her CSF came back normal if I recall but the IgG was present for NR1, which I guess means Anti-NMDA. But what do I know. Sounds extremely similar to my symptoms when I got Lyme bacteria from a tick bite. It came on pretty suddenly like you and the easiest symptoms were physical, fatigue etc... but the scariest and strangest symptoms were mental like you described. I also had the shaking, numbness, words coming out wrong, mental fatigue and I had that strange feeling (which you described well) of the world suddenly being desolate along with panic attacks (I never had them before the tick bite). Super strange, and the hardest part is no one understands what you are going through. I would get a lymes test if I were you, but don't research it TOO much online because unfortunately there are some wackos on the online forums. I remember trying to share my story of recovery, and everyone accused me of being an undercover doctor. Rest, relax, meditate, exercise and recover. Hope you heal up fast!

The bacteria doesn't, but an autoimmune condition can be caused by it which can last for extended periods for some people. Others won't have any symptoms from Lyme and their immune system will flush it out immediately. Same with a lot of these weird little problems, theyre exaggerated by some of our bodies. This is important to understand for a lot of people because it's easy to start blaming every little symptom on a disease that may or may not be causing them, years later. Hope you're on the road to recovery!

I told my dad once that I thought the world around me was hell, everyone looking so evil, and that I couldn’t figure out why I was being punished. You are clearing a path thru a medical system minefield that will help others suffering from similar brain/nervous-system inflammation related illness and undiagnosed illness. This process in itself will not lessen the suffering you still face, however I hope it will give you a context to persevere, for yourself and others who have suffered and will continue to suffer in the future from undiagnosed illness in a broken healthcare system. People search for meaning in all the traditional places, wanting to make a difference in the world, but sometimes fate brings the fight of our lives we never asked for or expected. Your story shows that how we confront a life threatening situation can transform a miserable unjust fate into a small but necessary step that helps others we will never meet. In the end that is something of value for you and your brother. I am of an age that has required me to advocate over many years for ailing parents in a dystopian American healthcare system and I am blessed with both resources and a brother who is an award winning primary care doc. Sometimes all that can be gained at specific points in the painful journey is to map the mines so the next family might get further along. You twins are doing that and more with your talent now to forewarn the rest of us and I think reduce the future suffering of others. Thank you both for that courage .... of necessity, if not by choice.

Don’t know what to say. What a remarkable story … not remarkable to endure but remarkable that you have endured to this point. I’m much older than you so I want to confirm your future is bright. I, too, had a mysterious psychotic break with reality about 15 years ago. Came outta nowhere, disappeared as suddenly. It last only 18 hours so what I experienced is nothing compared to what you and your brother have endured. But it was unsettling all the same and it resulted in a Bipolar diagnosis and bullshit over medication that I passively submitted to for 12 years. I finally called BS on the diagnosis and all the crap meds I was taking (benzos, SSRI, anti-psych, anti seizure, etc.) and took myself off all the meds. I am fine. What happened to me is unexplained to this day. I just wish I could have avoided the decade of altered personality due to the meds. That was far worse and costly than the psych break. Anyway, my story isn’t yours. Not nearly as bad. But I understand a little what you have gone through and I commend you and your brother for enduring, persisting, fighting, and reaching out. You have changed my existence by sharing what you have gone through. Best wishes…

You and your family's experience sounds unimaginatively horrifying. Your mental fortitude is an inspiration. Glad you’re still here and that things are starting to look better. Hope your recovery continues and only expedites itself.

On and on. Our brains are funny things, so sensitive to disturbances. I hope you and your brother can find out what's caused this so that you can get better treatment than just prednisolone.

Hearing you talk about this almost gave me PTSD. Went through something very similar in 2016. Labs, MRI, CT Scan and spinal tap. Nothing ever came of it. Eventually got better but it took about a year to really feel like myself again. Glad to hear you are on the mend 👍🏼
@waylonhowse47622 жыл бұрын
That's odd because this happened to me the same year it happened to you, 2016. Labs, MRI, CT scan, nothing. Neurologists (I've seen more than one) won't even listen to what I'm saying. They just keep throwing different drugs at me because I have a headache that never goes away. None of them work/help. Something is still wrong. I've had a headache and ringing in my ears since the summer of 2016... I just take BC powder and live with it as best I can.

Wow, what a compelling story. You had me in tears when the film played of you saying "I'm out of my mind." I'm so sorry you & your bro are going through this, and I'm happy to hear you've found some relief. I only discovered your videos about a month ago, so I didn't notice your absence & actually scrolled over that video many times. I thought it was probably about the market crash 😆 I am new to trading (and kinda bad at it 😔), and as the market started failing in April, I decided to try to learn options trading. You are a super intelligent and well-spoken educator, but I still haven't felt comfortable enough jumping in YET. I need to watch your options guide 2 or 3 more times. I've actually been taking notes! The way you have it all set up like a course and tell people when/where to find more info before moving on is extremely helpful, informative, and...easy. I just want to learn how to put without losing any more money. Not trying to dive in to lose another 5000 without knowing every angle!! Anyway, you've said it a few times, but you are very lucky to have such great support. Sounds like a tight family. If you didn't have them, you could have very easily succumbed to the suffering. I wish my dad were still around... Please know that steroids at this point is probably a bandaid. They aren't meant for long term treatment, I don't believe. I'm not a doc though. Please listen to them and get several opinions. You seem to be a brilliant trader and teacher, but they are qualified, even if some of them don't know how to simply LISTEN. But also, no one wants to hear that they exhibit signs of bipolar or schizophrenia disorders, but as someone who if becoming self aware (at 35) of a very possible case of bipolar or BPD or something, it's very surprising to me how much I just missed it or didn't see it until I realized the possibility. It is a stigma; but it's common and doesn't mean one with it isn't "with it." sometimes it's hard to see it from within, even if you're sharp as a tack. I'm NOT saying that you have these things going on though. In fact, it does sound viral, it could have been Covid despite the tests, schizophrenia doesn't come on like that, (I don't think, although there are intense "evil" hallucinations that are common), and you simply looked like you were sick as hell. Like, flu sick. I hope you both continue to improve and that you can further pinpoint lasting treatments. Disappointing to hear the intensely negative reaction to cbd. That's weird. But I sincerely suggest, if you are 1000% positive that you aren't schizophrenic, maybe very low doses of psychedelics will help. They are often the magic cure for things like this. *But only with serious caution and solid supervision!!* I should have gone to bed hours ago, but I was pulled in, and I could feel your pain. I wanted to share some words and thank you for the candid story but also, greatly, for the trading information that you share. It's invaluable. I have never donated on youtube but I am going to figure out how (especially if I can make some money back on options 😄). I'll get my Brave browser on that too. Thank you. Glad you are whole, man. Good luck, fellas!! Take care of yourselves. Justin L.

Hey Adam, What an incredible story and I am so glad you made it through the worse phase of this illness. For the record, this is most likely a case of Auto-immune encephalitis -- likely Seronegative (when all antibodies come back negative). You were probably suffering from focal seizures when you were having "breaks" in perception, olfactory aura, and visual deficits that were worse than your baseline mental fog. I'm surprised the neurologists did not perform an EEG as that may have clued them in. MRIs and Lumbar punctures can often be negative in these cases or "non-specific." This is a very rare condition and often misdiagnosed as schizophrenia (most common) or other psychiatric illnesses. It is difficult to treat and people may suffer from relapses but there IS treatment and it often consists of Steroids, IVIG, and immunosuppresants. I STRONGLY recommend you find a Neurologist who specializes Neuro-Immunology and management of Auto-immune encephalitis. You and your brother will need close long term follow-up but you WILL make it through this. You can message me if you need a referral. All the best and looking forward to many more videos.

I had some inkling of an idea what it must be like but this was far scarier than what I imagined. It really does make me wonder just how many people are stuck and left to suffer all due to misdiagnosis and whether there is some way to increase proper treatment to relieve a bit of pain in this world. If you know of anything please share. Glad to hear you are beginning to feel better and that you continue recover. Stay safe and take care.

This isn't too far different than my new struggle with temporal lobe epilepsy which has been absolutely horrifying. I don't convulse with my seizures so it was a long haul to get it diagnosed and many doctors were also very consistent with trying to psych diagnose. Those feelings of disassociation, psychosis, and anxiety that just pull you so far away from reality for breaks of time are really true hell and its absolutely terrifying. I'm glad you won your fight with the doctors to get proper treatment. Take care man and keep going strong, you got this.

Adam, take a look at the comment by the ED physician Reece about NMDA receptor encephalitis. Ironically, a lot of the symptoms you describe sound like a bad ketamine trip, which is a drug that blocks NMDA receptors (I am an MD anesthesiologist, so I’ve had patients describe this to me as well). The major point is that steroids made things better; if this was/is bipolar or schizophrenia, steroids would make those things 100% worse! I’m sorry you had to go through this. The medical system can really suck, especially when you have such a complex problem going on. Keep advocating for yourself and don’t let anyone talk down to you.
