I loved the bit with the intern (Boon was it?) right at the end as well; when someone asked "did he even care", and Boon, having just been stuck with Kelso all day turned around and just said matter of factly "yes, he did". No explanation required. Boon knew Kelso better than anyone else at the hospital in that moment.

Ken Jenkins does a great job as Dr. Kelso but this episode is truly a masterclass performance from him. Also, you finally get all the pay off to the continued humanization of Kelso and seeing what the hospital actually means to him and how it affects him. This is just an absolutely beautiful episode.

Hi Jess I just wanna come by that if I had one wish right now, it would be to have your whole commentary for every episode of scrubs, complete with medical insight! I just graduated nursing school but I still feel like I have a lot of work to do on the casual understanding of medicine. I've worked in a hospital as a tech for 5 years and now as a nurse, but there's still a lot of off-hand remarks in scrubs I still need work to understand. Your videos are like watching an episode of scrubs with an instructor, and I love it. Please never stop!

I can relate to the diagnosis smile. I was recently diagnosed with a rare syndrome that they suspected since my birth, but no one bothered to check. I was literally just smiling at the papers for the whole hour, wondering if docs thought I was crazy

A very connective episode. My wife had an atopic pregnancy which took the doctors a while to diagnose (3 to 4 hours of pain at a clinic) before going into surgery. I also have a fatigue issue that has not been conclusively been diagnosed. It probably is a bunch of odd things along with stress. My old doctor retired and the new doctor tested me for vitamin D and folic acid which end up being very low. So now I at least have a direction to see if supplementing those vitamins will help. I enjoy your show and how can someone not like Scrubs. 😅

I think this might be another contrived plot point. Doctor Townshend was like 80 or so and nobody forced him to retire. Kelso had to tell him to his face that he was out of touch and needed to call it quits. And he knew that because he spent all his free time at medical seminars trying to keep up. So why would people attempt to force him out at 65?

This is one of the all time great episodes. The whole show is a masterclass in blending comedy and drama, but this episode is easily top 5 for me in all 8 seasons (yes, I said 8. There are only 8. ONLY 8 (0_0))

Once again, a great episode.. could you please also review the episode my two dads from season 1. It talks about insurance and the dilemma. I heard from my doctor friends about the unethical ways of insurance fraudulent claims and fusging paperwork to accommodate patients without sufficient insurance coverage . But i would really like to see your opinion on that topic l. Please review my two dads episode from Scrubs season 1.

Yea, as someone with ME/CFS I can confirm that getting a diagnosis (especially after a long time without one) is already a relief. Now if only the insurance doctors would stop saying I can still work 40 hours a week... 🙄

Horrors seen ey? So first responder smoking at the scene. Doctor arrives "What the hell are you doing?" "Smoking?" "I see that, why arent you treating the patient???" "shes dead" "WHAT? I am the only one with the authority to pronounce someone dead! You are obligated to do anything you can until i pronounce her dead!" "Well she was 7 and if you want us to do CPR thats a bit difficult" "Uhu enlighten me how so?" "Well you see, her Torso is still in the car, her head however rolled down that cliff, we are still searching for it, so mouth to mouth didnt quite work" - guess who found a partially crushed head. That said i never had any issues with corpses or death thats why i was always called to remove the bodies to the cellar, dont know why but i genuinely never had an issues with corpses, whatever made it human left, and whats left is just matter, corpse, table, fabric - all the same to me

I think this episode is why every time when I am having a scrubs rewatch and someone in any episode is struggling to diagnose someone I always think or say it’s acute intermittent porphyria.

"Are they gonna leave the urine in the sun and diagnose porphyria?" five minutes into the episode. I've seen a few medical professionals react to this episode and that's a first lol.

Yes! I literally smiled and laughed when I got my autoimmune disorder diagnosed. My mom thought I was nuts, but the rheumatologist wasn't surprised at all. Then when she suggested cortisone shots, I was smiling and she was like, "I've never had someone smile about getting shots before." and I was like... I just want the pain gone. I was ecstatic that it was being taken seriously and that there was a solution.

with Scrubs you have to remember that its from 2001-2010, and most of the cases they base episodes on are from the late 90ies, in medicine thats along time ago... Kelsos first lost patient is proberly early 70ies

I've got mad respect for streakers. I'm not saying it's something folks should be doing, but I have overwhelmingly positive experiences involving them, and it takes courage to put it all out there like that. There was a pair of streakers at my highschool graduation, and one of them was a girl, so that really made my day. My kid, used to play that Imagination Dragon song, “Radioactive,” over and over, and it drove me so crazy, I decided if I ever get a lethal dose of radiation, during that “walking ghost” grace period, I'm to paint a big radiological hazard symbol on my chest, and go streaking with my boombox playing, “radioactive , I'm radioactive.” I'm going to record it too, like a music video. It'll be a hoot. My final hurrah, before the super icky, awful, real dying starts. I think getting my head taxidermied and mailing it to my ex-wife would be pretty funny too. Nobody tell her it's coming, so when she opens the box, it'll be a big surprise. If a person's head comes off, it's called a decapitation, right? If their butt comes off, would that be called a de-crap-itation? I was feeling pretty down because they have the pool chemicals out at Tractor Supply, so everything inside smelled like chlorine and outside smelled like fresh cut grass, so I was thinking a lot about poison gas and trench warfare. I feel in a much better mood now though. These are a whole lot of fun to watch, and I think you're a great reactor, Doctor Jess.

I do not know if this is a defense, but Dr. Kelso graduated med school in 1968. I am assuming medical imaging/testing has improved in speed and availability in the past 50 years. Also, med school may have still been teaching that single women do not have sex.

Love the scrubs reactions, and while you already said it in the reaction I just wanted to confirm that the knowing is a massive relief even if it's bad news. As a fat white guy with diabetes, a stent and cfs that transformed in to fibro it's always annoying and frustrating to have these wierd little pains or tiredness or whatever that keep impacting life but also you have no idea if they are serious or just a new normal so when you can go to the doctor and get a diagnosis, be it good, bad or just a "nah, these are nothing serious and you can ignore them if you can put up with them" then it means you have a plan for moving forward and there is not constant stress. For example, I was getting tingles and my arms and fingers for ages and it was annoying but seemed harmless and then one night I sat at my desk and my right arm instantly went to sleep. I got up, shook it out, sat back down and same thing happened. Went to a doctor the next day (yeah I know, should have gone to ER instantly most likely) and they were "we can't do anything about vague, indeterminate pain but I got an MRI referral, went to that and even before getting out of it they said they could see the issue. Instantly I was like "ok, there is something, now we can work out a plan and move forward"

Jess love your vids as always. Question. How common is it for someone to become obsessed with medical videos and shows after becoming disabled or living with a disabled person? I lived with and got engaged to someone born with spina bifeda and learned about wound vacs, picc lines and so much more. Even got a job as a home health provider. Then I became disabled. Since then, IDK I just find them so fascinating.

My grandma is in what's called long term care at the hospital my wife works at. It's essentially for old folks, maybe more for those with a lot of health issues but i don't think it's mandatory.

"that can cause depression" Uhmm Jess. it's Ted Buckland, His title/nickname is already the hospital's sad sack xD. He's been known to keep a gun in his drawer for when he gets really sad. Believe me, Ted has already passed that station. this was more at a time when jokes about this were dark but funny And while nowadays it may be a bit comfortable to watch but doing it this way also brought it to light

At the moment over half our acute beds are currently taken up by patients waiting nursing home beds, they aren't sick they just can't live at home anymore.

I was really privileged that way, I retired in april of 2019 on my own terms (at age 49) and was recalled in 2020 because I am an expert in a field that where all hands on deck was needed at the time, I made more in six months than I did for five years before I retired and didn't even ask for that. Now I'm retired again. Until the next crisis. I mean, I still do some work just for fun but I really don't need the money so I don't charge because I kinda feel guilty of getting the ridiculous number I asked for in 2020 that I was certain they'd refuse. Anyway, we got the vaccine out and that mattered and it's not universally tested so mRNA vaccines passed a great hurdle there and my work in the process is part of my legacy.

Dr. Jess, I read the book "The House of God" because I was told that the Scrubs writers read and used a lot of it in the show, and that it was very popular among doctors. Was that book on your radar in med school or after? In any case, I found it compelling, if a bit dark at times. Frankly, I feel the same way about Scrubs.

Hahaha I did see this episode way back when it aired - long before I knew what porphyria was (and definitely didn’t remember it was in a scrubs episode when I was in med school!) legit the purple pee thing is one of those key pearls you learn in med school 😅😅😅 scouts honor!

My experience working in hospitals for the last 5 years is that they're all long term acute care nursing homes now.

Why do they continue to mispronounce porphyria? You have it right -- even I have it right and I'm not a physician. There was a long running joke in House MD about how, during their differential diagnosis finagling, the mention is made of lupus as a possibility with the recurring consensus of "It's never lupus." And then, of course, towards the end of the series it was. -- just once.

Great question!! I think you’re right - there really weren’t pregnancy tests that worked that fast, or were all that reliable. Mama doctor Jones did a fantastic video a few years back on this subject! I highly recommend checking it out 😊

to be fair they might have given him 600mg ibp if they told him to take it every 4 hours :o i got 600mg ibp after my c-section along with some oxy that i really only needed at night for a few days :o

I don't get why anyone would want to work as a doctor under the discribed conditions. Having people die on your watch seems hard enough without the prolonged shifts and tight time schedules. Sometime during the decade long education one might have gotten that this is not worth the effort. But at this point, you're already to deep in to pull out.

Yep…. It can be brutal at times. But we do it because ultimately we want to be able to help people. The medical education system is a bit broken to be honest 😔

I had the same problem, for two years no doctor could give me a diagnosis. I was in and out of the hospital, had a heart catherization. I was tested by various experts including a trip to the Cleveland Clinic. I was finally the one who compiled all my blood tests and told my doctor who agreed I had/have hypokalemia. Now it is managed by large doses of Potassium and it is a relief!

I've never understood why age discrimination is ok when we've already made it clear that other forms of discrimination aren't ok.

Well there's a pretty good reason as to why when it comes to this, saw it first hand in another episode on what happens when one stays on too long and they need to learn new techniques. They don't have the energy for it and end up putting their patients at risk.

True true. But if there’s no other indication of not keeping up, simply discriminating based on age alone seems really really messed up….
