@@takethecannolis Actually the news video said that the surgery was performed at his surgery center, which is also his office, and does not have the proper accreditation. But more importantly, everything I just said was generally correct, so why are you being snippy? Reports have varied on the credentials of who was actually giving the medications and in the room with the surgeon. Some stories reported a tech, others have reported an “untrained nurse”. Getting into the question of; can an RN give sedation? Sure. Many breast augmentation surgies are done with just local anesthetic and conscious sedation. So depending on your state, your training, and your facility, you can occasionally see a well trained RN providing conscious sedation (or monitoring after the surgeon provides it), just as they would for a colonoscopy or certain other procedures.
@JimYeats Жыл бұрын
@@takethecannolis Ok, lets back up. The first poster said it was performed at a hospital, so I was just correcting them that it was performed at an outpatient location. Which it was. His office is literally by definition an “outpatient surgery center” because they are doing surgeries in the outpatient setting. Thats the normal term. Whether it is practicing appropriately or under proper accreditation and certification is another story. The original poster then referenced the surgeon hiding it from “everyone else in the hospital”, so I mentioned that for certain procedures all you would need is a couple of staff, again, depending on what you were doing. Secondly, you are aware and understand that this surgery was intentionally being performed NOT UNDER GENERAL ANESTHESIA. That is literally kind of the biggest point I was trying to make when you corrected me with regards to who was potentially in the room and who should be in the room. This surgeon was trying cut costs by doing this procedure using local anesthetic and conscious sedation. This patient WAS NOT INTUBATED or being given a paralytic. While breast augmentations are done most commonly under general anesthesia, there are many practices that use local anesthetic PLUS conscious or moderate sedation. This would be similar to a dentist and his hygienist doing a fairly major dental procedure using the same type of combination; you have a dentist and a couple of hygienists in the room and they are using IV conscious sedation. Depending on what medications you are giving and in what state a normal RN can absolutely administer certain sedative medications under a providers instruction. Other medications require the provider to push the medication and then the RN can monitor. So while this surgeon was clearly not practicing correctly, as we can see from this outcome, there are plenty of outpatient offices and surgery centers that are using this method and using a qualified RN and not using a nurse anesthetist or anesthesiologist, and that is perfectly legal and - in most cases - safe, although not necessarily preferred. The reason I delved into the qualifications of the staff and what sort of sedation they were using is because the news story here, and the untrained news personnel may not understand that you do not ALWAYS need to have a n anesthesiologist to perform certain surgeries and procedures. They were the ones who indicated that was incorrect, when in reality the bigger problem may have just been incorrect procedures, unclear staff training, and clearly…not a good response when something bad did happen. Edit: there are absolutely scenarios where an RN will administer propofol without any additional certifications other than what is needed to work as an RN (their license plus BLS/ACLS). For example, the Arizona State Boards opinion and guidelines state an RN can administer sedatives including propofol, depending on the level of sedation trying to be achieved. Propofol can be a tricky drug though, which is why Versed and Fentanyl are more commonly used.

OMG! This is terrible and completely avoidable. As a Registered Nurse who has done conscious sedation I don't know how many times, this did not need to happen. I see so many things wrong with the scenario as reported above. It is perfectly fine for a nurse to do sedation, not anesthesia unless you are a CRNA - Certified Registered Nurse Anesthetist. I have been in the OR circulating when the surgeon needed a lunch break but it was during a 7.5 hour procedure and either another surgeon was there assisting or an assistant. You don't leave the patient under anesthesia with a nurse. Listen people, don't go to the cheapest guy! Ask your friends if they recommend a surgeon. Look to the state health department to see if their surgery center is licensed. Call the Joint Commission to see if they are accredited, or some have a AAAASF another governing body for stand alone surgical centers. Look at the surgeon's medical license. Have any complaints been made to the state? Most of the time, everything goes as planned. But, there can be complications. Please do your homework.

When $$$ drives medicine, this is what you get. Remember, insurance companies will ok only the cheapest, most cost saving procedures first. So, even if the doctor wants to go to a more aggressive treatment asap, more often than not, he will be forced to wait along with you, hoping to save bucks for the insuring company, with a CEO making untold millions. Meanwhile, your cancer progresses to stage 3. I know. This was my experience. Greed is NOT good... in medicine.

What also blows my mind is that the nurse knew she wasn’t qualified to do the anesthesia. As the medical professional how can you knowing how important that job is and you know you don’t have the qualifications, think it’s ok to do that job? The extra study involved to specialise and the nurse thinks it’s ok? The American medical system needs to be looked at more closely to see where else this is happening because this was a ticking time bomb.

Plastic Surgeon's also have Surgical Techs. Unless the State guidelines have changed in the last 3-5 Year's.. In the state of Ca you do not have to be certified as a Surgical Tech to work in an OR be it Plastic Surgery OR or Hospital environment. Crazy to think that at least 5 year's ago it was not required that they pass the State Exam to become certified (Might be different in 2023 IDK) Regardless very uncomfortable to think that anybody working in an OR is not certified/Licensed.

@meridoughten9425 Жыл бұрын
My thoughts exactly! A breast augmentation should NEVER take that long. It is not a complicated surgery. The longer you're under anesthesia, the more your risk for complications afterward goes up :(
@meridoughten9425 Жыл бұрын
@@mairanass Depends on the person! A lot of women are able to mostly resume normal activities in about 1 week, but if you have complications or a lift in addition to an augmentation, recovery can take a bit longer. Personally I had a lift + augmentation and then some issues with the pain killers afterward, so it took me about a month to fully feel back to my normal self. But most people feel pretty good after 1-2 weeks, so just talk with your surgeon or the surgeon's nurse about your concerns. They will have the most info about your specific case :)

See folks especially ladies, this is why you don’t want plastic surgery unless if it’s absolutely necessary. Any surgery is risky even if doctors do it all the time. I’m not saying this happens often, but there is always a risk once you’re put under anesthesia. Not only the surgeon gets charged, but the whole team who worked on the patient should have been held accountable too. The nurse knew that was out of her scope of practice and should have refused to do it.

Many of these doctor office OR’s are licensed for sedation only, not general anesthesia. Sedation means patient is arousable as to protect their airway. Many of these office procedures extend sedation into a gray zone approaching general anesthesia.(unresponsive with airway protection needed) This is a problem nationwide. Outpatient centers which are licensed for general anesthesia are the safest for a few extra dollars. Multiple other doctors and anesthesiologist doctors around to help if problems. Also Dantrolene medication is available if patient develops malignant hyperthermia.
@Firestorm637 Жыл бұрын
Anesthesiologists and certified nurse anesthetists are experts at airway management especially if patient stops breathing and ventilation is needed as in general anesthesia.

The defense attorney’s argument is ridiculous. The doctor should have exercised much better management of his schedule so that he could tend to his OR and clinic responsibilities, if he wanted to handle all of that in one day. And that’s to say nothing of his inappropriate delegation of anesthesia duties to someone who wasn’t trained to assume those responsibilities.
