It being point blank is what saved rex. Basically when you normally die getting shot in the head its because the bullet bounces around in your head. The caliber and it being point caused the bullet to go straight through.

It's both it being point blank and the placement of the shot being not in the middle. The bullet probably just scraped the brain, leaving most of it intact. As long as the brainstem remains untouched and there's not a large portion of the rest of the brain destroyed, the survival change of an injury like that is bigger than one might suspect.

If you are wondering how he survived this, there are two ways. One, and this has happened a lot and is the most likely, because of the low angle at which the bullet entered, it was deflected by his skull and skittered past under the skin to come out on the front. This is definitely the most likely, as despite the blood, there is no bone or anything to suggest the bullet penetrated the skull. Bone can deflect bullets when hit at a low angle, and although it certainly has happened with the skull, it is more common with ribs. The second option is that the bullet somehow did penetrate the skull and exit the other side, missing the brain completely. The human brain is much smaller than the outer dimensions of the head, and the barrel of the gun was certainly lined up outside the area the brain occupies. This option is much less probable and much less survivable.

He put a .22 under her chin and shot her, the bullet traveled up, severed her right optic nerve, ricocheted around her skull, and exited back out her cheek area. She walked herself to the emergency room. I honestly completely forgot about this till now, but that is actually like the perfect example for how this is possible. I mean, this woman literally walked herself 2 miles to the hospital.

(SPOILERS AHEAD) That episode was brutal, and a part of me is sad that NONE of the people being killed actually died. Kate kept a double out in the mountains and the rest of them just kind of, survived. I felt like Kate makes sense because keeping a clone out in the mountains is ACTUALLY something she can do, vs just kind of, getting lucky. Ant girl should have stayed swallowed, rex could have died but I feel like he actually adds to the show, so resurrecting Kate and rex while keeping ant girl swallowed would have been better. After the shock of the episode, seeing all three of them Kate it out felt a tad bit lame and plot armor esc.
