Thank you Dr. Steven great presentation. After having a horrible attack of vertigo recently ( not the first time) it was never explained even though I know it had something with the crystals as some call it, I call it ball bearings are not inline. Anyway what made more intolerable was the nausea and not able to vomit due to a Fundoplication surgery a few months before. In any case I very much appreciate the matter of how you explained both the peripheral and central vertigo.

Hi I had vertigo 5 years ago room spinning! These past 5 months it’s been happening on and of 12 hours of spinning I went to ent everything was fine…they said it could be to do with my jaw but I’m not sure I’m now being referred to a neurologist cause of the bad headaches that come after my attacks I’ve had 12 attacks in one week!! What are or types of drugs that can cause central vertigo, as you touched on in your video?

Lots of drugs can do it. Seizure medications, for example. Just about any drug that gets into the brain (e.g, antidepressants) could do it. The rates are somewhat variable, I'm sure.

Hi Doc, so glad to see you again. My Girlfriend has had Vertigo for 4 years and no one can seem to diagnose why. Here is one take that I found quite fascinating. It is possible that she has some kind of dental condition that she cannot really feel due to a previous root canal. No nerve ending to detect pain, but it is possible that there is something untreated in her teeth/mouth which may be referring into her ear causing the vertigo. What do you think of that ? Thanks doc, always a pleasure watching your videos!

Thanks for your kind words, Bryan. Sorry to hear about your girlfriend's problem. I would think a dental nerve disorder would be highly unlikely to be related to her vertigo problem. The video doesn't really cover vestibular testing, which is probably a good idea for many people with chronic vertigo. Unfortunately, not every place has facilities or expertise for that testing. It involves a whole battery of tests (eye movements, a spinning chair, a moving platform, etc.) that can do a good job of localizing whether the vertigo seems central or peripheral. Might be best to look for a medical center nearby with a vestibular disorders clinic and vestibular testing facilities. Hope this helps.

Is it possible to have problems with both ears at the same time? I have been in and out of vertigo for over a week. Vomiting, diarrhea, room spinning, unless I hold perfectly still. I have done the Epley maneuver 2 times. It is not working this time. If it is both ears, how long do I have to wait until I do the opposite ear?

What could It mean when you only experience when sitting or laying still? No experience of vertigo when standing or moving, strongest when feeling sleepy. Any logic?

My grandfather died from Parkinson's. Me experiencing vertigo is really frightening.

I have had symptoms for many years now of the illusion of movement, positional movement affects it a bit but its always there to some extent and I have just learned to live with it and lead a normal life, quite active fit, squatting at the gym etc with no issues. Should I be worried about the more nasty potential 'bad actors' as you say, I would have thought if it was caused by something more serious I would probably know about it by now? Should I continue to push for this to be investigated. Doctors never seem to offer much help.

I agree that something which has been present for many years--and not gotten worse or caused new symptoms--is pretty unlikely to be caused by a sinister process. If you've been thoroughly evaluated by the appropriate specialists and nothing has been found, I guess I'd tend to keep my wallet in my pocket and continue going to the gym!

My vertigo started when I used earplugs every night, to shut out the city noise, long-term.

The other thing she does is she hunches her shoulders all the way up like she is gesturing "I don't know" and she will not put the shoulders down unless you tap her on the shoulders.

Hello, have you ever heard of vertigo presenting itself as a first noticeable symptom of MS or NMO? The vertigo was severe enough to wake me out of a dead sleep early in the morning and caused me to vomit uncontrollably all the way to the hospital. I was put on prednisone for 2 weeks and sent home and continued to vomit every time I moved a lot or opened my eyes.

Yes, I have had patients with MS attacks which caused vertigo. It's clearly not the most common cause of vertigo, but it can happen.

I was first diagnosed with MS August 2017 (I am in Ontario, Canada) then sent to another neurologist in November who said she wasn't convinced it was MS and suspected NMO. I have had 5 MRI's in the last year which found a large lesion on my cervical spine and a few "spots" on my brain. I have had my eyes checked for optic neuritis and they are fine. I don't have uncontrollable hiccups like someone with NMO may present with and have had the one antibody test that we have here to see if it could be NMO but it came up negative. (I am currently waiting for the government of canada to approve my neurologists application for me to have 2 more newer blood tests that will be sent to Oxford to check for NMO) I have been on gabapentin for the last year and recently started taking duloxetine for nerve issues...the gabapentin dulls the symptoms but my arms and hands have been affected 24/7 since all of this started last year and the duloxetine hasn't helped much other than taking away knee pain and gave me my personality back. As a neurologist what would make you steer towards NMO rather than MS in a patient of yours during your first few visits with them?

I went to HUP in Philadelphia, they have balance study center. Sure major University hospitals have such Divisions

I do not. Perhaps you could search for a medical center with a vestibular lab; that stands a better chance of having good people who deal a lot with vertigo.

Oof, vertigo sucks. I have epilepsy and monocular vision (20/500 with correction), and am hard of hearing. Lol so my balance sucks anyway. It’s annoying. I use forearm crutches which keeps me stable when walking but ugh. When it hits, it’s annoying. Your nauseous… when I lie down at night, yeah, sucks. Looking up, sudden change in turning my head, nope. Cerebellar nystagmus can make you REALLY dizzy too.

That type of vertigo is ca.ledthe syndrome e of de-embarcation. Have vestibular dysfunction with central vertigo, recently diagnosed with MSA-C, think is worse than the vertigo a,though its a component. No real treatments no cures. Neurology is an area of medicine in its infancy. 30 critical care ER RN.
