This has been on my heart this week - between working 1 on 1 with mentees and seeing the landscape online of new grad nurse practitioners, I see this ALL the time.
Feeling like you're the only one feeling this way. Feeling behind, not knowing enough, and then beating yourself up for it.
If this sounds like you, take a listen-- sometimes just naming and acknowledging what's ACTUALLY going on makes a difference in how you're feeling about it.

Wow!!! Im a dude, and I though it was sink or swim (either you thrive or fail). Honestly, I was just ready to hang it all up, screw the student loans and go back to being just a simple "worker bee" ED RN. This video hit hard; I've been an NP for about 2 years now. Thank You.

Liz, great video! I'm halfway through my 5th week as a new FNP at a primary care clinic. Your honesty is greatly appreciated,; more than you know. At 50 years old and with 15 years of various years of RN exprience, I sense a little bit of intimidation and resentment from my younger more experienced NP/PA peers. They are helpful when I ask questions, but I do not feel the support I know I will provide to others that will be in myt shoes. Like they do not remember how it felt to be fresh out of the gate. Much like I did at the RN level. I definietly feel lost and behind; wondering why I did this. Again, your candidness in this video gives me the hope to hold on and take the time I need to be safe and effective as a new provider. Sincerely Rob. S. CRNP

I quit on Monday, it was so toxic there, I didn't fit in with the cool kids. 6 different preceptors, no consistency, I was not learning a system to build from. At 10 weeks I was essentially given the option to resign or get let go..... they actually did me a favor as I feel as though a huge weight has been lifted off of my shoulders! Unfortunately, this was my pcp's office...was! Next chapter, primary care is possible, just not under those circumstances. Urgent care, here i come! Thanks again for your support! Rob S. CRNP

Thank you thank you thank you for this. NP with 6 months experience here and that feeling like you are never doing enough or ever caught up is so real. It’s hard going from the expert as an RN back to being the beginner again especially in a small practice where you feel like your resources are thin. I appreciate your channel so much!

Physician assistant here! I’ve been in clinical practice for 2 years and the complexity of clinical practice as well as the amount of mental and emotional energy that it requires is blows my mind! Thanks for the reminder that we are in this together, we are always learning, we are doing better than we think, and even the seemingly small things we do for our patients can make an impact. Thank you, you are amazing!

Thank you making this video! I am currently in the BSN-DNP program at Temple U and have 196 days left, not like I'm counting. Feeling not good enough to be an NP creeps into my mind a lot. You are an amazing being!! Thank you for your authenticity and heartfelt empathy for our special field of practice 💓 Working as a sole NP in a brand new NP clinic role as a brand new NP in primary care was insanely isolating, but I had a fabulous consulting who continues to be a mentor even though I left NP practice after 7 years. Mentorship makes all the difference and I will continue to tune into your channel. My position was defunded, that’s why I left by the way, and having not practiced for 3 years I had to let my license expire per our regulating body here in Ontario- but I’m still thinking of returning to practice and people like you help me remember why I loved it and how good it felt to really make a positive change for the women I cared for. Thank you!

Thanks for making these videos. You have a really humble and light energy about you/your videos. What do you think about someone going to FNP school (goal of working in adult primary care) with only one year of specialized experience in maternity? Do you think it's necessary to get med surg or ED experience first?
@RealWorldNP

I think that's what a lot of people recommend, and I know for me it was super helpful having worked in the hospital with high acuity adults before becoming an NP, but I also work with master's entry students and clinicians who are excellent. I think trust your gut! I knew that working any longer as a nurse in the hospital would make me unhappy, so I decided to apply to school after 3 years as a nurse, and ended working as a nurse at 5 years once I graduated.

Hi, I came across your channel because my wife started being a nurse practitioner a couple of months ago. She is a brilliant and caring person and I love her to death. She has been feeling severe anxiety and waking up in the middle of the night thinking about her patients. She signed a contract to the doctor's office she currently works at for 2 years and thinks she needs to just "go through this and stick it out" but I want to know is there anything that can at least elevate her anxiety at all? She doesn't think she is cut out to be a NP and feels so much pressure at her job and to herself. As a husband, I worry about her so much and want to know is there anything I can do for her? I know this is an old video so I hope someone can help me with this. Thanks again and great channel!
@RealWorldNP
@RealWorldNP Жыл бұрын
The fact that you care so much about your wife is wonderful and will be very helpful as she progresses through the transition. Encourage her to find a mentor or someone who she can talk with regularly - this is important! That person can help talk her through diagnoses and labs and everything else that might be challenging right now. There are a lot of videos on this channel about transitioning into practice that can be helpful to her. There are also a lot of free resources on this channel - cheat sheets and links to places that can help her with her practice. Tell her to get on social media and find some NP groups that might offer guidance and support. And, most importantly, hang in there! Let her know we are all cheering her on and this will be tough, but she will get through it. She will eventually feel more confident in her work - I got there, and she will too!!!

New NP here (haven’t started working yet) Do you order routine blood work every three months on patients who are on antihypertensives and metformin( like a BMP and A1C q3 months) ? Also Are there any guidelines you can point me to that state to order routine blood work and how often for certain meds a patient is on ? Thank you!
@RealWorldNP
@RealWorldNP 3 жыл бұрын
Unfortunately there really aren't any guidelines that I know of that talk about routine lab screening aside from the USPSTF guidelines. Typically we order too many labs in primary care I think, it really should be condition specific aside from those types of guidelines. Another thing to note about screening guidelines. USPSTF is one option, but you get to decide for yourself if you prefer USPSTF vs another guides (ADA, etc) and which you align with for which patients. Makes it confusing unfortunately! But yes, typically patients with DM and HTN I'm monitoring their labs every 3-4 months depending on their baseline, how their conditions are doing, and if there have been any big changes for them. Hopefully that helps!
