I Speculate that this sub-level is a hotel because if there is a reception desk it has me thinking this level is a hotel however I think the hospital is combined with this level which is why there might be that room there and with that makes me believe we’re gonna have to preform surgery on the Koax laying on the bed

i feel like the sublevel will be in the pit path of level 23, the one where you encounter cruelest and fall down a dark hole and feel like the sublevel itself will either be: habitable zone (due to the slightly industrial themes of the pit, with cranes, metal, concrete and such, but it might be more likely it'll come as a normal level in update after this one, the update which includes level 94) Cave system [the mines] (The sneak peak we got today, this could be explained as the pit being some sort of excavation site, and the levers it features) and last being the hotel (perhaps when you fall down you end up in a broken area before the hotel itself and have to travel across elevators another option could be to enter the level you have to find an open door in level 23 which would lead to a room with the elevator leading to reception desk room)
