My brother passed away at the age of 39 from Acute Respiratory Failure with diffuse alveoli damage, due to aspiration of gastric contents during a seizure event. The para aortic lymph node was enlarged. Any insight into why this happened? He experienced flu like symptoms the week of his passing. Chest pain the day before his passing. Nasal congestion, sore throat. Worst he’s ever felt. Underlying COPD, Emphysema and Asthma. Cyanosis on extremities. Difficulty swallowing and collapsed on the 21st May down the stair, a month before his passing. He says he blacked out. Smoked heavily, 7 hubblys per day. Equivalent of 380g of nicotine over 2 years.

When plateau pressure is high, won't you have to decrease PEEP instead of tidal volume in avoidance of barotrauma?

Yes, reducing PEEP could reduce the plateau, but the reason you would institute a lung protective strategy (i.e reduction of tidal volume (4-6 cc/kg) and increase the rate) is because your plateaus are 30 cmH2O or greater. Typically in ARDS, we need the added PEEP to keep the patient oxygenating, so backing off the PEEP may not be possible or it may cause the lung to derecruit. So, we'll drop the VT and implement permissive hypercapnia.

Pao2 and Fio2. It determines how much of a percent of pure oxygen a person needs to be on to maintain Sao2. For teference room air is 21%.

@@thezerowulf2046 Just to piggy-back on your statement: remember also to divide by the percent written as a fraction, not a whole number. ie. PaO2 = 50 mm Hg and FIO2 = 30% (50 / 0.3 = 167 P/F Ratio)
