There are so many good things about these videos. The content is super helpful and well organized and I have learned so much, but I have to admit that my favorite part is the random 'turns of phrase' that Zack will say. Absolute nuggets of gold hidden throughout the videos - 'You have 32 teeth, but 30 of them are in your pocket'.

I'm 34. @ 32 healthy combat veteran. I had MRSA endocarditis. For 3 straight weeks I felt unbelievably sick. Migraine shit and puked @ Same time. Woke up 1 day and couldn't move (1st stroke) family brought me to hospital where they ran blood tests and started iv antibiotics. During the iv I felt loopy and my face got droopy. They brought me for catscan and mri confirmed I was having a second ischemic stroke. Went in for emergency thrombectomy. After that had ekg and echo which confirmed the leaflet on my mitral valve. About 3 weeks later I had open heart surgery replacing my mitral valve with a bovine @ yale new haven hospital. I'm 34 now still have left side deficits. Endocarditis is no joke!

My cousin has large vegetation in mv. I'm surfing for the information and I found you. My cousin has exactly the same symptoms as you said now I understand it from you. But in the real situation it's not easy to evaluate the problem because the symptoms are generally occurred in many organs of body. We/doctors spent 3-4 months to find out, echo 4 times in 3 hospitals, MRI 2 times (brain and heart) and finally TEE and one of the doctor team asked about the dental decay and bingo all symptoms can relate with it. Thanks for the great knowledge it surly inspire many people around the world to be a great doctor because of your channel.

Sooo I'm a dentist and not all dental procedures will require antibiotic prophylaxis in high-risk patients. Most, but not all. Like totally edentulous patients getting dentures are typically fine without it. And generally, if you aren't sure, we think it's better to err on the side of doing it rather than not doing it and running the risk of an infection. Even then, there's been some debate in our field on if we are even preventing anything (for example, flossing your own teeth may push bacteria into the sulci of the teeth, which ultimately drain into the inferior vena cava.) We do it anyways because the benefit outweighs any risk the antibiotics could cause. Just sharing some fun facts. Presentation is absolutely amazing. You should seriously consider teaching. Your KZbin resume speaks for itself!

I have been in the nursing field for more than 30 years. Your lectures are more informative than any textbook or physician. You have missed your calling and should be teaching physicians. Thank you for your comprehensive lecture. Just to add a point that for typical organisms blood cultures ideally should be 3 , from separate venipunctures and spaced over 30 to 60 minutes and if 2 out of 3 is positive it is considered a major criteria. However for atypical organisms we should look for persistently positive blood cultures which means either 2 positive blood cultures drawn at least 12 hours apart or 3 positive blood culture in which the first and last samples were collected at least one hour apart. Important finding for a prolonged episode of infective endocarditis , is the presents of splenemegaly and hypersplenism features like thrombocytopenia and anemia that can be seen .
