Correction @27:50 , the ventilation INCREASES as we go down from the Apex to the Base, but the perfusion increases SIGNIFICANTLY more. Since Perfusion increases more than Ventilation as we move to the base of the lung, the V/Q ration decreases at the base

Zach, I'm a Term 1 medical student and I just want to stop by and tell you how much I appreciate all your videos. I'm on my last week of the first term and we have our CPR2 exam coming up and your videos have clarified so many points in their lectures. A year ago my father died from a massive pulmonary embolism. As you can imagine, its very difficult for me to listen to these concepts and understand just how much he went through during his final moments. He's my sole motivation for becoming a doctor. I hope I can make him proud in the years to come but as of right now I've been able to accomplish that thanks to you and your channel! I look forwards to watching many more of your videos throughout my medical school journey. Thanks again!!!

27:15 !Ventilation is higher at the base of the lungs (in an upright position) due to gravity pulling on lung tissue which increases pleural pressure!

I feel like you should have emphasized that the smooth muscle in the arterioles contracts and relaxes because capillaries dont have smooth muscle and so the illustration can be misleading.

At time 27:30, you said that ventilation decreases from apex to base, but actually it increases from apex to base of the lung. Perfusion also increases from apex to base, but much more significantly that we have a lower V/Q at the base.

There is an error: ventilation is higher at the base than at the apex because of increasing compliance. The V/Q ratio is biggest at the apex because at the apex: Q higher is relatively to V. V/Q ratio is lowest at the base because V is higher comparatively to Q.

The ventilation being talked abt is in the alveoli and not outside the alveoli..the ventilation will be more at the base but that phenomenon is applicable to the outside of the alveoli

Correction: Type 1 Alveolar cells make up 40% of the alveolar cells in number and Type 2 make up 60% of the cells in number, however the type 1 alveolar cells take up approx. 95% of the space and type 2 approx 5% of the space Correction 2: Nitric Oxide causes smooth muscle relaxation by activating guanylyl cyclase, increasing cGMP concentrations in the smooth muscle cell, activating cGMP dependent protein kinase which in the end phosphorylates and activates Myosin Light Chain phosphatase which leads to smooth muscle relaxation. It is not dependent on K+ efflux.
