Kurt was a tortured soul, and this song is exactly how he felt, isolated and alone. His life was more tragic than any of his songs. This makes me so sad. Heroin only made his life worse, but the tough love bullshit that they threw at him just fed into his abandonment issues that had plagued him since childhood. If anyone you know and care about is struggling with or strung out on anything, tough love is fucking bullshit. Don't use that failed policy on anyone you really love. It could kill them.

It depends on how far gone someone is, their mental state, and if they talk about wanting to either quit or kill themselves to family and/or friends. Know this, though. With all the knowledge about hard drugs like heroin crystal meth and some prescription drugs, whoever does them even once isn't doing drugs recreationally. They're almost always done by people who have deep emotional problems and think they need to escape.
