this would be so helpful if nurses ever did it like this lol , student here everyone does it differently very difficult to learn when every nurse is different some use dots with numbers, some numbers and others just numbers.

But in blood pressure column it says systolic blood pressure. Should we mark diastolic pressure also

Hello @Anuradha De silva - yes, you chart/mark both the systolic and the diastolic blood pressure, but only the systolic blood pressure counts towards the score. So, for example, if your patient's blood pressure is 105/85 , you would still draw the arrow like in the video (02:51) and the score would be 1 (not 4). Hope that helps. What if we placed a dot instead of A. Will that count as inaccurate?

@iamthehamham If you're referring to Consciousness, no that would be fine. If you're referring to Air or Oxygen? Then A is expected for Air. Hope that helps and wishing you the best of luck with your NMC OSCE preparation and exam.

Please for a pre op patient with a news score of 1 because of BP of 101/60 is I hour monitoring frequency good? And should I escalate in the column for escalation?

Hi @Ifeoma James - your NEWS chart documentation needs to be correct and for an overall score of 1, the monitoring frequency is 4-6 hourly. Yes you would escalate. Hope that helps and happy studying!

Thanks for your response. I was told that the patient needs to be monitored hourly since she is going for surgery.

Hi @Ifeoma James - you can verbalise that you'd monitor your patient more frequently than the NEWS chart suggests, but your documentation needs to be accurate. The monitoring frequency sheet is attached to the chart, so please follow the guidelines. Happy studying!

Please if you have a total score of 4, at the bottom end do you write Y for escalation?

Are we supposed to write the figure when the score is 3 ,as in if we get a respiratory rate of 28 ,are we to write 28 or just a dot is okkk

In which score/s will you put "N" on Escalation?

What if the total NEWS score is 7 and above? Do we write and fit " 15mins" or should we write "Cont." in the small box for monitoring frequency? Thank you!

CONT or the applicable symbol would both be accepted, although in 5+ years, we've yet to have a candidate who's had a patient with a NEWS score of of 7+ in their exam. Wishing you the very best of luck in your exam.

So its either cont or C if ever? Thanks. Another question, in plotting for the BP. Dots only or V and inverted V can be used for systolic and diastolic bp? Lets say in a hospital setting.

Y is for Yes and N is for No. You would usually escalate ie. mark Y for any NEWS score above 0.

Hello Anyamele - could you please clarify what you mean by a bad score and what you mean by a good score? If you mean what score requires escalation, then we would consider a score above 1 to require escalation with the medical team.
