I always show the start of the run selection because when you launch the run the game asks you to turn off any accessibility settings for a better experience.Also since you say "you have at LEAST 3 on" could you point them out please?

Yeah sadly the game is unplayable for me as well after the patch. Hopefully ND fixes it soon because it never was this bad before.
