Contrast timing is horrible --- should have some contrast in there somewhere - can't see any in the jugulars or the carotids.

Yes. A CT scan without contrast could also detect it

what do you say about the sinus , almost always there is mucosal tickening ?

Good point, it is unusual to have completely clear paranasal sinuses, there is usually at least minimal mucosal thickening. In these cases, it could be reported as "no significant mucosal disease". It also would not be entirely incorrect to just state that the sinuses are clear. You do have to be careful though, sometimes you can have invasive fungal disease with very mild mucosal thickening. In this case, you need to evaluate for signs of chronic mucosal disease (bony changes), and evaluate surrounding fat/soft tissue for signs of disease. Patient history helps with this. If there isn't obvious history, but there are complaints of headache with no clear cause, I like to look at the sinuses more carefully.

@@immane75 It's possible, but I would be looking for a little more of an osseous defect/lucency surrounding the crown.
