NL you were taking "random" damage because of rubber cement + multidimensional baby. Your initial shot would hit an enemy, and while bouncing around it would pass through multidimensional baby, which creates another shot that sometimes wouldn't hit anything.

It was the combination of rubber cement and multidimensional baby. You would win the room with your tears and then eventually after the room was finished the tear would bounce around and hit multidimensional baby. Same thing happened with tammy's head. None of the tammy's head tears matter for this challenge but the second one of them bounces and hits multidimensional baby that tear matters.

If you miss, you get hit, it's that simple. You don't get to fire all six cylinders in your revolver at the target, hit only one, and say "I'm a crackshot!"

From the wiki: Taking any item that multiples the number of tears (e.g. Mutant Spider, 20/20, Monstro's Lung) is not recommended, as Judas has to hit with every tear to not take damage. Similarly, taking The Parasite or Cricket's Body is not recommended, as the split tears also have to hit enemies for the player to not take damage. Afterbirth† Indicator.png Be careful when picking up any active Book items, as picking up 3 will transform you into a Bookworm, periodically giving you 2 shots. Taking Tammy's Head or Sad Bombs is not recommended, as they release tears which count towards your hit/miss. Since they release multiple tears at a time, they will almost unavoidably cause tears to hit the ground. Taking Rubber Cement is recommended, as hitting a wall won't deal damage to you, and hitting the ground won't either if an enemy is hit first. Proptosis allows Judas' shots to miss without dealing damage as long as they don't hit a wall or object.

NL, the game isn't made around the challenge. One of the aspects of the challenge is to not take items that could potentially make you lose, like having MultiD and Rubber cement.

You almost had it diagnosed; I think it was YOUR shot going through multi-dimensional baby and bookworm activating causing 4 shots to appear, some of them missing and making you take damage. And on occasion, your familiars KSing you and making you take damage. I dunno what you thought would happen taking Tammy's Head though, that one seemed obvious enough a bad choice, especially taking in account multi-dimensional baby would multiply some of those shots too but hey that's why you're NL! You make the runs entertaining to say the least. =P

Once you realize it's your shot hitting an enemy, passing through MDbaby and creating a brand new shot that never hits an enemy, or book worm & MDbaby creating 4 shots and only 1 to 3 hitting an enemy, you kind of just say "Oh yeah, that's reasonable." If you don't realize that then I guess it seems unfair.

It looks like you took damage because your shots were hitting the floor or the enviroment BEFORE hitting enemies, but because of rubber cement, they kept bouncing and killing enemies, hurting you first when your tear finally hit the floor at the end of the bounce.

Weigler yeah, but starting this challenge with chocolate milk, a sometimes-piercing shot, isn't fair, because the leftover damage that pierces through dead enemies shouldn't cause the player to take damage. I think that's a developer mistake or oversight, because there's no way to avoid damage, even if you're shooting perfectly.
