Sorry to hear about your condition. I’d combining Dr Esselstyn’s recommendations with time-restricted eating. The idea is to eat in a 8-10 hour window, with consideration given to circadian rhythm , I.e. eating most of your calories earlier in the day. Read the work of Dr Satchin Panda for more info and good luck!

I agree with Imran Hussain, long fasts between meals helps your body repair itself also fasting for days, though this isn't what Dr Esselstyn teaches, if I can recall correctly to chew on greens several times a day to release nitrate oxide into the bloodstream to help open the arteries. He could be right, so far in trying low Carb (keto) Intermittent fasting, no sugar, I'm 56 male have had very high hi BP 200/120 for over 20 years, take no meds, currently my BP averages 150/90 providing I keep to a disciplined lifestyle try to stay away from bad people that cause stress or other situations that cause stress, in the event I get into fight or flight mode bp goes straight up again to very dangerous levels. Life is difficult and does kill you in the end so get right with Jesus (truth) 1st, worry about family, health money etc. next. These are stressful times (cv19 lockdown) and will affect most people if not dealt with acceptance and thankful mindset. We are all on a journey of learning and growing.

I would argue that you may not have chosen and eaten enough foods which can be filling, called starches: Potatoes, beans, rice, whole wheat bread, whole wheat pastas. It's important to track the food by weighing and entering in a calorie tracker so you know you get enough calories. This is a common problem. I started doing this to shrink my enlarged prostate, which indeed shrunk a bit within 6 months. I do ingest plenty of olive oil, which is also propagated by eg. Steve Gundry. The standard reply about gluten is to avoid it if you have diagnosed celiac disease or diagnosed gluten intolerance, otherwise they say it’s OK. Try to get organic gluten products so that the grains don’t have pesticides.

I don't think it's very responsible for a doctor talking about curing CAD not mention the role of diabetes and advocate for eating grains. You can say what you want about meat/dairy vs no meat/dairy. That's a personal decision. But to not discuss the complete picture of the causes of CAD and the role of metabolic disorders and insulin resistance is inaccurate.

Study title: "Olive, soybean and palm oils intake have a similar acute detrimental effect over the endothelial function in healthy young subjects" Authors: Christian F Rueda-Clausen 1 , Federico A Silva, Manuel A Lindarte, Cristina Villa-Roel, Elieth Gomez, Roberto Gutierrez, Carlos Cure-Cure, Patricio López-Jaramillo Abstract Background and aim: Currently, more than 30% of the caloric intake in the Colombian population comes from vegetable oil consumption mainly by the ingestion of deep-fried foods. Recently, it has been reported that unsaturated fatty acid rich oils have a beneficial effect on the endothelial function. Nevertheless, it is well know that the deep-frying process alters the chemical composition of vegetable oils and can produce adverse effects in the endothelial function. Objective: To evaluate the acute effect of the ingestion of large amounts of olive, soybean and palm oils, fresh and at two different deep-fry levels, on the glucose and lipid profiles and the endothelial function. Methods and results: Ten healthy young volunteers were included in the study. After performing a baseline evaluation of cardiovascular risk factors and drawing a fasting blood sample, subjects were exposed to a randomly assigned potato soup meal containing 60 mL of one of three different vegetable oils (olive, soybean and palm), either fresh or at one of two different deep-fry levels (10 and 20 fries, respectively). Flow-mediated vasodilation (FMD) was performed in fasting conditions and 3h after the intake of the oil rich meal. Furthermore, blood samples were taken at these stages for the lipid profiles and plasma glucose determinations. All the meals resulted in a similar acute endothelial impairment (FMD decrease of 32.1%, confidence interval [CI] 95%, 28.0-36.2) and postprandial increase in triglycerides (27.03%, CI 95%, 20.5-33.3), independently of the type of oil ingested (p=0.44) and regardless of its deep-fry level (p=0.62). No correlation was found between endothelial impairment and postprandial triglyceride increment (r=-0.22, p=0.09). Conclusions: No difference was found in the acute adverse effect of the ingestion of different vegetable oils on the endothelial function. All the vegetable oils, fresh and deep-fried, produced an increase in the triglyceride plasma levels in healthy subjects.

It is the case that those on a vegan diet live shorter lives than either a vegitarian or omnivore one, so this is not conclusive. However, a vegan diet includes crisps and oil, so your point about would make a difference. There is also psychology involved here, that cabbage 4 times a day would make me thoroughly miserable and depression/stress is highly inflamatory. This is all food for thought and thanks.

If you are on keto and are trying to eliminate good healthy carbs from your day-to-day diet, you are only painting yourself into a corner. Unbeknownst to you, your cells are becoming congested with lipids. And soon, insulin will knock on their door as usual but, get no reply. This will be the pre-diabetic stage. Then you’ll one day out of the blue go in to full-blown diabetes.

Cholesterol is in your entire circulatory system. Why is there clogging of the ARTERIES close to the heart and not in the veins or capillaries in your nose, ear or knee? This is because the pumping of the heart can cause hairline cracks. Blood pressure is highest in this area. These hairline cracks are repaired by a sticky substance LP(a), to prevent from getting worse. Why don't animals have cardiovascular disease? Almost all animals produce large amounts of vitamin C in their liver. On average 35 mg/kg body weight. So we have to take at least about 3000 mg of vitamin C ourselves because humans cannot make vitamin C. You get strong collagen tissue, strong connective tissue, strong arteries. Hairline cracks no longer form and arterial plaque is removed and burned in the liver. Repair with cholesterol is therefore a consequence of a deficiency of vitamin C. So do not take statins, but vitamin C in a high dose: at least 3 grams, better: 6 to 10 grams Source: Cardiovascular disease and vitamin C (Dr. Rath Foundation)

This i a fact i have testes this both with doctors taking 3 tests 1 week apart, all with huge variety 250, 180, and 230. I have also done it with home total colesterol tests. It can vary quite much just from one day to another. This with zero changes in my diet . So what does this tell you. Well you don't have to be very bright to understand it!

Best treatment for atherosclerosis: Vitamin C Why do only coronary arteries clog with cholesterol and not veins or small capillaries? If "high cholesterol" is the cause of clogging your arteries, why doesn't it clog the arteries in your finger, your nose, and the smallest capillaries in your toe? Scurvy = No vitamin C in your diet. Symptom: Your blood vessels break and you bleed to death. Think of the sailor of the past. Scurvy of the heart = Just enough vitamin C from food. Symptom: Arteries around the heart are not strong enough to resist high blood pressure. Damage is repaired with cholesterol LP(a) to prevent worse. After years of repair, your arteries become clogged. A guinea pig can also not synthesize vitamin C by themself, just like humans, and needs supplementing 50 mg vitamin C every day and weighs only 1 kg. 1 orange = 50 mg vitamin C. Do you really think that 75 mg vitamin C for YOU is enough? What is your weight? Enough vitamin C supplement of at least 3000 mg. per day gives strong and flexible arteries. Cholesterol is not needed as a repair agent. Cholesterol in your coronary arteries is broken down and burned in your liver.
