Feel like many things would change- Orochimaru wouldn't have any support or leaf intel, no uchiha massacare, shisui would be alive, he would be there during the 9 tails attack perhaps even saving minato and kushina, he might be there during orochimarus attack as hiruzen's guard perhaps and save hiruzen's life perhaps, he woud be a kage level ninja and would take part in the 4th shinobi war.

I mean the ending would be different, Naruto wasn't raised in Konoha, he won't have his dream of being Hokage, maybe he would still stay in Rain Village and help them as the way Nagato did, along with Konan, while Sasuke stays in Konoha and they still keep contact. Also, the two village will be companion and Sasuke acts as the messenger
