Feel like many things would change- Orochimaru wouldn't have any support or leaf intel, no uchiha massacare, shisui would be alive, he would be there during the 9 tails attack perhaps even saving minato and kushina, he might be there during orochimarus attack as hiruzen's guard perhaps and save hiruzen's life perhaps, he woud be a kage level ninja and would take part in the 4th shinobi war.

Also, the two village will be companion and Sasuke acts as the messenger

Assuming for whatever reason they didn’t need to take it out, Kurama wouldn’t have had any shred of respect for Naruto he would never share his power and would do anything he could to get unsealed and set free.
