It helped me to understand -how to choose relevant information from a case note -how to write the purpose of the letter - how to make simple,compound and complex sentences -usage of present perfect continuous ,past tense,present perfect tense - usage of since and for - structure of a letter - when we put comma in a sentence.

My learning experience from this vedio I got an idea about the criterias of letter writing. •how to write Introduction paragraph For letter writing we need to use ISBAR method. Structure of the sentence How can make simple, compound and complex sentences. In simple sentence-subject+verb+object In compound sentence, two independent clauses joined with a coma and a conjuction. In complex sentence, one independent clause and one dependent clause. If first writing dependent clause we need to put comma but if writing first independent of clause no need to put comma. I got an idea where to use simple present tense, past tense,present perfect & present perfect continuous How to find out relevant & irrelevant data. We need to ask two questions 1)is this case not relevant to the current situation?yes or no 2)will this case not affect how the reader continues care for the patient? yes or no If answer to both questions-no,that means this is irrelevant. If answer to these question- yes,then that is a relevant data. Where to use since & for For specific (starting)time mentioning we can use since. For we can use in the case of duration. Father wonderful session I never knew I could attenmpt writing in oet which was thought to be impossible but with your strategy nothing is difficult. Just keep posting sessions like this and definitely we will be making use of your strategy and try to be relaxed. Definitely we will reach to an extent where we llgo live. Today's my learning experience In complex sentence dependent clause and independent clause are their. In case some sentence starting with dependent clause its compulsory to add a comma after that write an independent clause. No need to put a comma when sentence Start's with independent sentence. Thank you Father 🌹🌹🌹for uploading the video. I was not able to attend this class, so I was waiting eagerly. Your session gave a vivid idea regarding the writing test. Now, it is easy to organise the letter and construct sentences. Expressing sincere gratitude to you Father because previously when I will get a letter I was so confused. Now, it's pretty easy to start writing and finish within time. Also thank you for suggesting Grammarly and Google photos. They became my companions in letter writing.🙏🙏🙏

habitual action/ routine action that continue to the present // we can use simple present tense eg- He is a regular smoker . in order to emphasise the duration and still continues //we can use present perfect progressive - He has been smoking one packet of cigarettes per day for the past 10 years.

I am sharing my learning experience from this vedio. WRITING STEP BY STEP * Assessment criteria PCC²GOL² *ISBAR 1) How to choose RELEVANT case notes? Ask yourself 2questions 🌹Is the case note is relevant to the current situation? YES/NO 🌹Will this case note affect how the reader continues care for the patient? YES/ NO If answe to both question NO= NOT RELEVANT If answer to either question - YES= RELEVANT (include in your letter ) 2) What to do with the aspe🙏of past medical history that are either normal or not very important?? ILLUSTRATION *No smoker *No known allergies *No further hair loss *No current medication *No significant medical history *No surgical history *No reported drug use *No pregnancies SKILL: SUMMARISE 🌹The patient's medical history is unremarkable. *⬆️smoking, smoke 30/ day * ⬇️ alcohol on weekends. Drinks 3wine glasses at night. 🌹 She is a regular smoker(30cigarette /day) and drinks moderately on weekends. INTRODUCTION Task 1) Whom you are writing to? What is their job role( designation )? Locate : Writing task ( last portion of the case note) Task 2) What's your role in the patient's care? Locate: in the beginning of the case note ( soon after the date) 3) What type of letter? ( Purpose of the letter) Locate :discharge plan( most recent information, just above the writing task) Introduce &Identify IPC²P 🍬Initiation 🍬Details of the patient 🍬Chief complaint ( main issue ) 🍬Purpose INTRODUCTION Initiation( referal letter) - I am writing to refer Details of the patient - Ms Peterson, a 56-year-old single lady Chief complaints - Who was diagnosed with...... Purpose - She requires your advice and management. INTRODUCTION I am writing to refer Ms Peterson, a 56-year-old single lady who was diagnosed with chlamydia trachomatis. She requires your advice and management. SENTENCE STRUCTURE 🌹SIMPLE SENTENCE 1)She is a regular smoker 2) She drinks moderately on weekends. 🌹COMPOUND SENTENCE She is a regular smoker and drinks moderately on weekends. CASE NOTES 1) weight :86kg, height :157 cm 2)Peterson concerned about weight gain of 8kg over last 12months SENTENCE She has gained 8kg over past 12months and currently weight 86kg. SYNONYMS Post operative = recovering from/ after Further care/ follow-up care Palliative care/ hospice care Was diagnosed ( short tirm diseases) Has been diagnosed (long term diseases ) Continuity of care/ on going care ? Diabetes= is suspected to have PARAGRAPH STRUCTURE ASSESSMENT CRITETIA- COHENSIVENESS Write the sentences together with the help of transitions to make a cohensive paragraph. But - contrast Apart from that - transition And- conjunction present perfect progressive Have/has+been+-ing formof the verb Eg: has been getting Has been taking When to use present perfect progressive? * Repeated action that started in the past and continues to the present. Eg: Mr James has been taking pain killers to manage the pain. SINCE 🌹For a specific (starting ) time 🍬since 5pm 🍬since morning 🍬since monday 🍬since April 🍬since 1st June 🍬since 2014 FOR Duration of time 🍬for 10 minutes 🍬for 3hours 🍬for 5 days 🍬for 6 months 🍬for 2 years 🍬for 30seconds STRUCTURE OF A LETTER 1) date 2)Address 3)Dear/Re: 4)Purpose - introduction 5)purpose expanded -most imp. information 6)Other paragraph - secondary information 7)Other paragraph -social /famly history 8)Request - conclusion 9)sign off USE OF PAST TENSE 🍬When describing an event in the past which is resolved. Eg: Mary gave birth five weeks ago. 🍬with diagnosed Eg: Mary was diagnosed with pneumonia and treated with antibiotics prior to discharge last week. ( pneumonia is now resolved ) 🍬when the situation is ongoing but is stable. Eg: Mary was diagnosed with diabetes in 2008,for which she takes regular medication USING PRESENT PERFECT 🌹Use of present perfect with diagnosed. 🍬When the condition is unstable. Eg: Mary has been diagnosed with asthma and prescribed an inhaler. She will return to the outpatient clinic next month for a rewiew of the inhaler's effectiveness. (Treatment is going on- not resolved) 🍬When the condition will be news to the reader 🌹when the reader already know the patient and so you are telling them something new. 🍁COMMON WAYS TO USE COMMAS 🍬compound sentences 🍬complex sentences 🍬Introductory phrases 🍬Transitional phrase 🍬List of ideas or items. COMPLEX SENTENCES 🍬One independent clause+one dependent clause. 🍬Independent clause contains a verb and is a sentence by itself. WHICH CLAUSE FIRST? 🍬If the independent clause come first= DON'T USE COMMA. 🍬If the dependent clause come first= USE COMMA. COMPOUND SENTENCES 🍁Independent clause+ independent clause 🍬two independent clause 🍬joined with a comma and conjunction SIMPLE SENTENCES (subject + verb+ object= SVO)
