I am writing regarding Mr X, who has signs and symptoms suggestive of space occupying lesion. He is being transferred to you and now requires an urgent assessment and management. Today, he was admitted to the hospital following a fall due to severe headaches, resulting in him injuring his left arm and right leg. On examination, he was semiconscious and had a slurred speech and in addition his pulse and blood pressure were high.

Hi i appreciate ur work i ve taken ur correction service. Can u tell me one thing we have to write urgent visit 1st but if in case notes it is written the urgent visit is worsening of previois symptoms how will we write thn ? Should we merge the two nd thn write previous visit again.

If it is an emergency or urgent reference, your first body paragraph should contain the details about the specefic emergency situation. Such as clinical manifestation and emergency action. Following paragraph you can mention the previous visits.. like... during initial visit he had so n so...
