Hai ma'am. Mr John Samuel , your patient, who has been diagnosed with urinary tract infection is being referred to you for further assessment and management Ma'am could u pls tell me is this sentence is correct or not,??

Hi, it depends on what the case notes say. However if you use "your patient" then it seems strange to say "being referred to you". Only use "your patient" if writing to the patient's current GP or nurse.
