I’m a therapist too! I think the car going under represents Billie and her brother Finneas diving into their career/fame together. There’s no escape, constant pressure… genius metaphor! She’s driving and he’s along for the ride. It’s really a complex song despite being relatively low key.

When Billie is asked about lyrics in her songs, she almost always responds, "well, what does it mean to YOU!" Although her songs often seem deeply personal (and they often are), she has always said she frames her art to be meaningful to the listener, not just her. While Billie had been suicidal in the past, and I think the lyrics in this song are a callback to that time period, this song - to me - is a reference to that one person who will stay by your side no matter what, even the impossible situations (represented by a car at the bottom of the ocean). @Smurfrise pointed this out and I think they're exactly right. So the real question isn't "what did Billie mean by that?" - it's "what does this song mean to YOU?"
