Thank you for the surgery But , in most of cases of stage 5, things doesn’t go smoothly as this. As, after you remove the lens and due to the intensity of fibrovascular proliferation, you can’t tell where is the retina. Thank you

I agree completely. This is so far my best case. In my experience, most complications arise when trying to dissect the fibrovascular membrane from the ciliary body, either because of bleeding, or because a retinotomy is created.

@@hiteshjoshi6422 surgery is the only option in stage 5 but how much vision baby will get that can be seen only with time as baby grows....

It was stage 5 closed funnel so a pars plicata approach was impossible.

Hi everyone I deal now with a stage 5 ROP since 6 months. One eye is lost and the other one has been operated one time by myself without success because of very gradually cloudy cornea during FVM dissection. The next approach will be open Sky vitrectomy. Could you give me some tips and tricks for this 18 months monocular baby ? Thanks a lot Dr AISSANI

Hello Dr. Aissani. Sorry for the delay in replying. I hope that the surgery was a success if you have already performed it. Here are some tips: 1. It is important to create the partial thickness sclerotomy before corneal trephination, in order to do it while the eye has adequate IOP. Trying to do it when the eye is hypotonic is exceedingly difficult. 2. I would advise dissecting the FVM with a blunt instrument as much as you can. That way you reduce the risk of a retinotomy. 3. When you remove the cornea, place a reference suture at 12 o'clock (or some other reference mark so the cornea is oriented correctly when you put it back. 4. A very thorough preoperative discussion with the parents, in order to set adequate expectations. Failure rate is extremely high.

I am sorry that your daughter was diagnosed with this disease. Each case is different, and management varies accordingly. My suggestion is to take your daughter to a retina specialist for an evaluation. Sometimes surgery can be beneficial, and other times it is better to leave the eye as it is to avoid further damage.
