do you have to ask all of the option, under a specific section like for instance the characteristics of the pain( quality).. do you need to say '"mr Jay, how does the pain feel like, is it dull crushing , burning ...........( mentioning every decription?) or you can provide a couple of them?

it only means some time two similar skills are given together. Anyhow you will have to total of 4 clinical skills.
