I’m assuming that this was bacterial pneumonia since he mention that swabs were negative for viral pneumonia and culture(s) came back positive. But how does she get seen by a HCP and prescribed medication without noticing that she was hypoxic. They could have easily used a pulse ox and noticed that her oxygen saturation was low. I would an independent review of antemortem visit notes to determine what vitals were measured. Make it make sense
