No, she's not the "same girl from the beginning". The woman singing was Kawakita Nao. She's the Maximum the Hormone drummer and guitarist Kawakita Ryo's older sister. Nao was a big inspiration and role model for both Murata Tamu, Nemophila's drummer, and Hirose Akane, the drummer for BAND-MAID. (Names are in Japanese order. Family name, followed by given name).

So, a bit of context for this one: this song is called "Let’s Talk about Menkata Cottelee" and is a bit of a f you letter to their fans. You see, their main vocalist and guitarist, Kawakita Ryo, was diagnosed with some health issues and had to go on a strict diet, resulting in him losing a ton of weight (you can see him both before and after in the video). Thing is, a lot of "fans" started complaining, stating they don't like his new look and demanded he go back to his old physique (represented by the "fans" happily reminiscing about "the good old days" at the start of the video, transitioning into them quiet literally vomiting bs complaints from their mouths all over their social media feed). So they released this song, which basically states "go screw yourself; I'm happy being thin and healthy and I'm not going back to being in pain just to appease you". Respect to the guy for standing his ground.

Guitarist Maximum the Ryo-kun's trademark was his obese body from the beginning of his career, but he lost a lot of weight in 2015 due to dieting as part of treatment for a lifestyle-related disease, and this song was written in response to Ryo-kun's ruined public image. It has become an answer song to the criticisms received.
