In this video, metadata_storage_name is not checked while customizing target index but in the next slide(part 5) when you perform the search via Search explorer, how did the metadata_storage_name appeared that has a file name(which is IPL.pdf)?

Please mark metadata_storage_name as Retrievable, then you will get them in the search results

In the Search Index Fields, mark the field type as "Collection(Edm.String) ". Hope this helps, let me know in case of any other questions.

If i want to use azure sql db as datasource, how can i use sql query to fetch the data?

You can use Azure Sql as datasource, but for the query you have to use the Azure Search queries.
