Patient Testimonial - Type II Achalasia Cardia treated successfully with POEM surgery

A female patient from Shillong, Meghalaya presented with difficulty in swallowing, tickling feeling in the throat. She was having progressive symptoms since 2 years associated with vomiting, regurgitation of food and loss of appetite.
⚫ Diagnosis:
Patient evaluated with UGI endoscopy, it had shown dilated esophagus with mild resistance at GE Junction, significant contrast retention seen during barium swallow esophagus diffusely dilated with smooth narrowing at GE Junction.
⚫ Treatment:
The patient underwent the POEM surgery for Type II Achalasia Cardia treatment. Further oral gastrografin radiography (OGR) study done on POD2. The patient tolerated liquids well, also free of symptoms, like vomiting, regurgitation of food, and discharged in stable condition.

রোগ নির্ণয়:
ইউজিআই এন্ডোস্কোপির মাধ্যমে রোগীর মূল্যায়ন করা হয়েছে, এটি জিই জংশনে হালকা প্রতিরোধের সাথে প্রসারিত খাদ্যনালী দেখিয়েছে, জিই জংশনে মসৃণ সংকীর্ণতার সাথে বেরিয়াম গ্রাস করা খাদ্যনালীটি বিচ্ছুরিতভাবে প্রসারিত হওয়ার সময় উল্লেখযোগ্য বৈপরীত্য ধারণ দেখা গেছে।
চিকিৎসা:
রোগীর টাইপ II অ্যাচলাসিয়া কার্ডিয়া চিকিত্সার জন্য POEM সার্জারি করা হয়েছে। আরও মৌখিক গ্যাস্ট্রোগ্রাফিন রেডিওগ্রাফি (ওজিআর) অধ্যয়ন POD2 এ করা হয়েছে। রোগী ভালভাবে তরল সহ্য করতেন, উপসর্গ মুক্ত যেমন বমি, খাবারের পুনর্গঠন, এবং স্থিতিশীল অবস্থায় ছেড়ে দেওয়া হয়।
