As it's an island there should be military presences on the island as well. Why don't you spawn some military troops by the checkpoint and the security shacks there on the island with some Humvees and helicopters since there is a runway there too? Spawn a few police units and sorta makeshift hospital with a couple of ambulances and for serious condition patients they should be medevac'ed to the city.
