Now that unending dispair gives both MR and armour, I can see it being a good tank item on fiddle and lissandra. Champions that want to dive into the middle of the enemy and have self healing in their kit. Healing is fair more effective when you have the HP and resistances to live through the damage you take so that you can heal it back.

An ult that is up fairly often, deals ~800 dmg level 6 to everyone in range (and it's not "800 split", it's "800 per target"). So you can easily jump in to counter gank and kill everyone pretty easily, especially if you're unseen and fear because of it. Once you get malig and liandry's, you just run around the map and look for gank angles. Or use your W to push lanes. One thing Pekin didn't show off this time - you can use your W on jungle camps to heal back a ridiculous amount (just cancel the drain before you kill the camp so you don't steal your jungler's camps). That's because Fiddle is a Jgl main role so he drain gives extra healing against jgl monsters. Kind of stupid that they don't check to see if you're jgl or not (since only one person can have smite and only one person can ever buy the smite items. Or just check to see if you have smite period (then it works for all gamemodes).
