Thanks for your video, it is my scenario, out customer specified 25 dimensions as critical, whether we should add all those dimensions in PFMEA

Hi Nithyakumar, if one dimension is contributing for fitment or function, that dimension's failure modes (over size & under size), effect of that failure mode, cause, current controls and further actions (if required) to prevent that cause and to detect that failure mode are definitely to be added in pfmea.

Hi Marudhupandi, i have experience in turning. Rod hardness will vary batch to batch in Raw material. If it is more the turning tool will get blend soon and the OD will increase if the operator not noticed the tool condition.

for example a defect is failed to found in inspection stage but it would be found out in the testing area due to missed inspection (eg-mobile assembly) so severity will be high with respect to that stage and the detection will be also very high at that stage so it means RPN increases . how to solve these?

Hi Siva, Severity (S) will not change at any cost and will be decided based on effect (see severity table for rating). Ex. If a brake wire assy fails in 2 wheeler it will lead to accident and severity will be 9 or 10 such cases. Even though you are increasing your inspection to robotic sensors for ensuring the assembly of brake wire parts, the severity (accident due to failure in brake wire fails) will not change. But your detection rating (D)will be reduced may be from 6 to 2 or 1 (due to robotic sensor / inspection). And occurrence rating (O) may be reduced by proper prevention controls in assy process and may be reduced from 6 to 4. So, RPN is the multiplication of S X O X D and may get less RPN due to reduction in O or D only. Hence, at any cost severity should not be reduced and remains same. But the selection of severity rating based on products end application is important. Hope you have understood.
