Vitiligo: a breakthrough therapy - The introduction of SCENESSE® in vitiligo treatment

Vitiligo is an acquired depigmentation disorder that is characterised by a chronic and progressive loss of functioning skin and/or hair follicle melanocytes. Melanocytes are found in the epidermal layer of our skin and produce melanin, the pigment that gives colour to our skin and hair. The loss of epidermal melanocytes in vitiligo leads to a loss of melanin pigment which leaves the affected area white.
Vitiligo has been described as one of the most psychologically devastating diseases in dermatology. The condition is a chronic relapsing disorder and new depigmented areas of skin can form without warning, having a profound effect on a patient's quality of life. Discrimination, embarrassment and low self esteem are just a few examples of what a patient can endure with the disfiguring effects caused by vitiligo.
Phototherapy, mainly narrow band UVB (NB-UVB), has emerged as a mainstay of repigmentation treatment in individuals affected by vitiligo. NB-UVB utilises a localised light source to activate melanin in vitiliginous lesions of the skin. This therapy is known to effectively suppress the local immune response and accelerate the maturity of melanocytes in the area around hair follicles, which act as melanocyte reservoirs. This process leads to activation of melanin (pigment).
This is where Clinuvel's first-in-class drug SCENESSE® will play an important role.
The novel therapy narrow-band UVB and the clinical response by patients, provides an excellent indication that melanocytes are present in the vitiliginous lesions of patients. After years of research, the clinical and academic community and the scientific teams at Clinuvel are well aware that where melanocytes are present, there are melanocortin-1 receptors available. And where there are melanocortin-1 receptors, there is an opportunity for SCENESSE® to activate the melanocyte.
The belief is widely supported that SCENESSE® will act as a chemical agent to the melanocyte in combination with the physical stimulant, narrowband UVB phototherapy, to repigment the skin of vitiligo patients.

If I understand the video, It seems to me that SCENESSE won't have any effect on autoimmune Vitiligo. If SCENESSE stimulates the Melanocyte to start up again, than it wont have any effect on Melanocytes that have been completely destroyed and removed by antibodies. Am I correct, or am I not understanding the video?

Hi Charlene, The risks of UV exposure are indeed of concern, however research suggests that NB-UVB causes significantly less damage which may lead to skin cancer. It is, of course, advisable to undertake this therapy under close supervision of your dermatologist.

try 311nm narrowband lamps. They work for psoriasis and vitiligo. They don't totally heal, but they bring relieve. I fight Psoriasis for 10 years and these lamps seem to work real nice for me. They claim to bring relieve for Vitiligo, too. Might be a shot worth trying!

At present this is a separate entity to the Australian TGA, but work is underway to merge the two organisations and harmonise applications. Im just telling my experience . I do not think there is any risk of making it worse than it already is , especially if you have Viti on your face . And mood, Tyrosine makes me happy . i have vitiligo. i've had it for 20 years, im 23. it's been repigmenting for me since i was 16. according to this video the type i have is nonsegmental. i used to have patches on my knees, they're now completely filled in. i still have it on both feet and my eyes and some of my fingers and toes, those are clearing up slowly. my eyes are taking the longest to clear up and it's not as fast as other parts. im not sure what i've been eating or doing to cause this change. i have stopped eating fast food entirely and cut down almost completely on sodas, but i'm not sure what that has to do with it...

It is wonderful that there are great strides in trying to discover a cure/ treatment for Vitiligo, but I feel that the most favorable cure/treatment should be devoid of lasting or damaging side effects. I feel that this plan for cure/treatment is very risky because the result is possible overexposure of UV rays causing damage to tissue in patients that already lack protection against the harm of UV rays thus could cause possible cancer in the future for these prospective patients care for.
