You got some good points there. Based on what I’ve researched from TvTropes, Fake Peppino used to be a human named Bruno who runs his own pizzeria, but he got abducted by Pizzahead and mutilated him into a deformed clone of Peppino; due to his almost identical appearance.

Probably at the end of the game, Peppino “adopts” Bruno | Fake Peppino as his younger brother and allows him to help out in the kitchen out of kindness and consideration. It may offend his estranged brother Maurice with the thought of being replaced by a molten-looking skinwalker, but it won’t matter to him.
