First think about what areas you would like to see improved Lorna, talk to staff and clinical governance leads in the area about what needs improving to narrow topics down. Local managers and clinical governance/ risk management leads will have data on incidents - talk to them. To review evidence base look on healthcare databases. NHS websites have data on clinical incidents too.

It originates from the Deming Wheel reference below: Deming, W. E. (1982) Out of the Crisis. Cambridge, Massachusetts: Massachusetts Institute of Technology for Advanced Engineering Study. A more up to date reference from the Demining Institute below for 2023 and you could use either or both references: The W Edwards Deming’s Institute (2023) Enriching society through the Deming philosophy: The PDSA cycle. Available at: deming.org/explore/pdsa/ (accessed on 16/11/23). I cannot put italics or bold in these references so adhere to your University referencing guidance. I hope it helps.
