Thank you doctor for sharing this video. It is okay for endodontic treatments but, not for an oral radiology center. We always have scatter radiation. I prefer using this portable device with lead apron and leaded wall. Always using the ALARA rule, " as low as reasonably achievable" :)

A radiology center is a different story. It’s not practical for high volumes. But at the same time, keep in mind that ALARA is just a suggestion and not backed by any specific science/research about actual doses or even chronic exposure to low scatter doses showing clear risk. The American Association of Physician physicists are in agreement with that statement but because the US is a big litigious country they always cover their back by making recommendations that does not leave them on the hook if some later shows the opposite of what they said! Oopsie! I'm sorry! I didn't mean any disrespect! I didn't know it's available in Mexico. That's great. It's not FDA approved in the US and Canada yet so it will be a while long for here. Cheers! :)

Hi Dr Ali Nassae I am a great fan of you. I am from Bangladesh, a General Dentist, posted as a Dental Surgeon in a Govt hospital. I have a question about my Rextar Portable Handheld X-ray machine. I am using it for the last one year without any backscatter shield with Eighteeth Digital sensor. I always try to keep the Exposure duration below 0.10 second to ensure more Safety. Is the short exposure tine really cause any benefit regarding radiation safety? or it is the same as long exposure time? thanks in advance.

Hello Dr. Nasseh, I am a new general dentist graduate who has been watching your videos for a very long time and using the original EndoSequence system since I started practicing post-schooling. From my understanding the NiTi rotary files are not heat-treated in this system and are therefore not as flexible. I would like to know if this system can be used on moderate to severely curved canals safely without a high risk of separating or ledging/perforation. I am currently using the EndoSequence system and have only had mild-moderate root curvature but it seems as though the shaping of canals is relatively straighter which is to be expected since they aren’t heat treated. I have used heat-treated rotaries during dental school which is why I am concerned with this. Thank you for your help.

Alec, you need to use the EndoSequence Blend protocol. It combines the non heat treated EndoSequence files with the heat treated EndoSquence Scout files. Then you have a system that can approach all kinds of canals. Go back and see two videos ago from this one. I will also have future videos on the Blend concept. Good luck! :)

@@AANasseh Thank you for the timely response Dr. Nasseh. So just to clarify if I had the original EndoSequence system (not ESX) with the small, medium and large procedural packs can I just purchase the smaller ES Scout files such as the 25, 20, etc. and then use just the same EndoSequence 30, 35, 40, and a few other larger single files from the procedural packs I purchased prior? Essentially I could use the same files from the procedural packs but with the new blend technique you described in the videos and I will no longer perform the Procedural Pack crown down technique (described in the original Endosequence technique videos) but instead will either go up or down the steps after the expeditor file just like you demonstrated. Please let me know if this is correct. Thank you again.

@@alecgaroian1661 Yes definitely! The original EndoSequence protocol by Drs. Koch and Brave is excellent and a very robust system. All you need is to add a few EndoSequence Scout files in specific cases that are very curved and you'll be totally fine. I would say use the sizes 17/04 and 17/06 as I showed in a recent video. You can also have the EndoSequence 20/04 0r 25/04 as well in some other cases. cheers!

@@AANasseh Thank you again. I apologize I am just a bit confused. In most of the Blend procedure videos I saw that you essentially mentioned using a 30.04 rotary as the expeditor and then going down to a 25, 20, 17 scout file with 04 taper in case of the smaller/curved canals. If I am using the original ES system should I use one of the 30.04 rotaries in the procedural packs and use that as my expeditor? Also should I no longer be cycling through these packs as mentioned in the original ES technique, and instead just do single file up or down until WL is reached just like you showed in the ES Blend videos?

@@alecgaroian1661 Yes. That's the Blend Protocol that Dr. Koch and I recommend; But obviously there are multiple ways to blend these files together. The conceptual idea is to use the Conventional ES Files in the coronal 2/3 of the canal where it's straight and the heat treated EndoSequence Scouts in the apical 1/3 where it's curved. You can use this combination anyway you want based on this concept. Good luck!

Thanks. No different than any other xray head. A lead apron is recommended, even though it's really not helpful. I've found it takes longer to explain that it's not needed than just putting it on! LOL!

Might or might not pose a hazard. But I think doing X-ray on pregnant women isnt a good idea. If there is a problem with the fetus and she searches where the problem might be and she sues you for taking that xray, it might be very unpleasant for you at minimum. And then it might not matter whether the xray caused it or not

@@martinchrom4444 I completely disagree. People can sue you for anything. If they can demonstrate scientifically that the xray caused the fetus to have problems then you may have a point. There's no research showing that. They only reason people don't use it is because they are afraid. My point is that you can use more harm by not taking an important and useful xray and deprive yourself of vital information that could help you save the tooth and end up with an iatrogenic problem only because you were afraid. You should educate your patients, not let them dictate treatment plans. Medicine is not served on a restaurant menu. People can't choose standards of care based on whatever irrational fears they have. At the end of the day, if something goes wrong and you have done an error because you didn't take an xray, it's your patient's attorney who will ask you on the stand: "But you are the doctor... doctor! If your patients said don't wear gloves or wash your hands would you listen to them?!!!" We should know what's right and wrong and stay within the standards. We shouldn't let patients dictate standards as they are not held up legally to those standards, we are! Cheers!

For endo yes. But if you take a lot of full mouth X-rays you’ll have to convince your assistant. I definitely would recommend a dose badge for them to know they are safe. After a few months they realize it is actually safe and won’t need it.

@@ZACY1234 Sorry I don't. We used to use them a long time ago but not since. You can always ask your supplier rep. There should be a few companies around I've seen them advertise. Just do a search online.

​@@AANasseh its still audible but your audio signal seems like clipping (even though you are right i ve got pretty cheap/horrible speakers for now but problem is obvious). Lowering the input level may be? If you are using adobe premier, it has stock software inside to check audio levels. Endodontics is my profession, audio / video production is my passion. (I was looking for something to criticise in content but couldnt find hehe) Cheers.

@@aksisoylenemez LOL! Well, it's actually normalized to a higher output than my usual.That's probably what you're hearing. Since you know you're an audiophile let me tell you that I only use pro gear: Sennheiser MKE2 Lav. with a Sennheiser AVX Wireless system sending the signal to a Mix Pre 3 by Sound Devices. It's an expensive set up for audio only. Today I decided to normalize and set my output level pretty closer to 0.0 db. instead of my usual -3.0 db. If you have weak speakers/iPods that will clip the sound since it can't handle the loudness. Good feedback though. Many people listen with AirPods or weak speakers that can't handle this loudness level. I will go back to mixing at -3.0. Thanks for the feedback! :)

@@aksisoylenemez Yeah. A shotgun mic is obviously better than a lav. But it's not practical for the kind of run and gun shooting I do. In my studio, I do use a Sennheiser MKH 8060 into a sound devices mixer. But when I'm sitting at my dining room table it's too much of a pain to set up. Lav is easier. Thanks for the feedback. It helps to know some people's audio is distorting at this level. 👍
