I did one for England v Italy for a number of years as a knockout tournament where I played almost every match! It involved teams from the top 2 divisions in England and Italy but also involved teams from Switzerland to fill the other 2 places in the first round. The first round was England v Italy or Switzerland with half at home and half away but the rest was a free draw. I still have highlights of this on youtube
