Prof Bert Vogelstein : Cancer Genes & Their Implications for Patients

I would first like to talk abouthow many cancer genes there are. For example,there are only threedriver genes, KRAS, TP53 and SMAD4responsible for abnormal cancer growth in pancreatic cancer patients.Onlyabout 120 to 200 driver genes have been discoveredfor the 26 major cancer types. These genes represent less than 1% of the 20,000 genesin human. Nosingle mutation is responsible for any cancerand at least three sequential mutationsare requiredto cause this disease. It also takes decades for these three or more sequential mutationsto occur.When ProfessorSuzanne Topalianasked if we could think of a reason why patients with melanomas andlung cancersrespond to immune checkpoint inhibitorsbut patients with other tumors do not, we looked at these tumor typesandfound that melanomas and lung cancers have many more mutations than other cancers. We furtherset out to do a clinical study of mismatch repair-deficient colon cancerwhich has 10 to 30 times more mutations than colorectal tumors,to see whether this theoryabout a link between the number of mutationsand checkpoint inhibitors holdstrue.Results fromthe studydemonstrated that not only colorectal cancers but many other cancer types also had responses to immune checkpoint inhibitors as long as they havea large number of mutationsand the FDA confirmed that this discovery marked a new paradigm in the development of cancer drugs.Unfortunately,evennew drug therapies won’t always cure cancer.All therapies will work better if cancersaredetectedat an earlier stagewhen the tumors arestillsmall. Screening is important for early detection of cancer andI believe thatwithin the next five years,cancerswill be detectableat an early stage in many patientsby some sort of liquid biopsythat we or others have developed.I strongly believe that the two avenues of research on earlydetection and new drug therapies must continue in order to reducethesuffering, morbidity and mortality from cancer.
まず、がん遺伝子がいくつあるのかについてお話させていただきます。例えば膵臓がんの患者では、がん細胞の異常増幅を促すドライバー遺伝子はKRAS、TP53そしてSMAD4の３つしかありません。２６の主要がん腫において、ドライバー遺伝子はおよそ１２０から２００しか発見されていません。これは２万個ある遺伝子のうちの１％も占めていません。また、がんは一つの変異によって引き起こされるわけではなく、３つ以上の変異が起こると発生する病気です。そして、この３つ以上の変異が起こるまでに何十年もかかります。スーザン・トパリアン博士に頼まれて免疫チェックポイント阻害薬が黒色腫と肺がんには有効でも他がん腫ではあまり有効ではない理由を探ったところ、黒色腫と肺がんは他がん腫と比べて変異数が多いことがわかりました。この仮説を確認するために、通常の大腸がんの１０倍から３０倍変異の多いミスマッチ修復欠損大腸がんにおける研究をさらに実施しました。研究の結果、変異が多いがん腫であれば薬が有効であることが証明でき、FDAもこの証明をがん治療の創薬における新しいパラダイムであることを認めました。残念ながらこのような新しい治療薬が上市された今でも、がんを必ず治癒できるわけではありません。早期段階でがんを発見できれば腫瘍量も少ないため治癒率を上げることができます。がんの早期発見にはスクリーニング検査が重要で、今後５年くらいでリキッドバイオプシーなどの検査方法が進展すれば多くの患者を早期発見することができるかと思っております。がんの苦しみ、罹患率、そして死亡率を軽減させるには早期発見と新しい治療薬、どちらも開発する必要があると強く信じています。
