Project Zero 1-3 are known for their scary idle scares/eastereggs, where you will leave your character idle for several minutes, after which something creepy will happen:
Zero 1 - Bloody Hands
Zero 2 - Woman’s Shaky Head
Zero 3 - Screen Goes Static
Even Zero 4 had one of these, but it pales in comparison to this even MORE secret and RARE easteregg, where this creepy mask will appear at random as you exit the inventory/pause menu.
In this video I tried to brute force it, eventually getting it at the end after filming for 4 1/2 minutes, but I then tried going for another round where I filmed for 15 minutes and nothing happened. Now, that may not seem like a lot of time, but imagine how many times you can pause and unpause during 15 minutes, so in a regular playthrough where you don’t pause all the time, this easteregg is extremely rare and on my own first ever playthrough I only ever saw this ONCE, scaring the bejesus out of me.
Note: This is the original Japanese Wii version with the English fan patch applied. The recent internally released remaster has a slightly different interface and pause animation, so I do not know if this easteregg still works on that version. I haven’t tested it.
