Harry's POV. This is set during Deathly Hallows, more specifically after Ron leaves and up to where Harry and Hermione escape from Bathilda's house and Hermione nurses Harry back to consciousness.
Ever since I first heard this song, I knew it was perfect for Harry and Hermione. You're free to interpret it as a perfect, solid friendship or as romantic love.
Harry essentially reflects on how Hermione is the only one who has always, always been there for him, no matter what. After Ron leaves, he and Hermione are devastated, but then Harry realizes she won't leave him and that they still have each other. As they travel to Godric's Hollow together, visit his parents' grave, and fight off Bathilda Bagshot and Nagini, Harry reflects on how Hermione has always been there and is still there, and how much they mean to one another.
Basically, they've made a promise of love and friendship, to stick by one another all of their lives.
