Thank u very much for your detailed explanation form harrison.but westermark sign is dilation of pulmonary vein just proximal to the thrombus,and Hampton sign is wedge shaped pulmonary infarct near the pleura.

What about antiplatelets, Beta blocker(for right sided HF) to stabilize heart. Your line of treatment is based on old Treatment guidelines. Current treatment guidelines have changed

As there is Pulmonary HTN that leads to right sided heart failure. IV saline can contribute to exacerbate right sided heart failure. I think you should correct yourself or me by logical explanation
