I thought we wanted to give EPI asap and no longer after the 1st shock? Is that just with asystole/PEA?

Yes, you're right. The approach to administering epinephrine varies based on the cardiac rhythm and the associated Advanced Cardiovascular Life Support (ACLS) algorithm: Asystole or PEA (Pulseless Electrical Activity): Epinephrine is given as soon as possible, typically after the second rhythm check (approximately 2 minutes into the code). Then, it's administered every 3-5 minutes thereafter. VFib (Ventricular Fibrillation) or Pulseless VTach (Ventricular Tachycardia): The priority is immediate defibrillation. Epinephrine is given after the first or second defibrillation attempt if VFib/VTach persists. After that initial dose, epinephrine is given every 3-5 minutes as needed. It's essential always to follow the most recent guidelines set by resuscitation councils, such as the American Heart Association, as these guidelines can evolve based on new research and findings.

Depending on the organization, there are still many monophonic defibrillators in practice. During my travel nursing years, I ran into some monophasic machines had to shift my way of thinking during cardiac arrest codes ☺️💜

Hi, I am a new grad RN taking ACLS for the first time this coming weekend. No idea what to expect so I have been going over your videos. Do we have to "act" out these individual scenarios like the one in this video or are we getting a written test on these? I understand there is something we have to physically do as part of examination I am just not sure what it is..

Great question IamBellator ☺️💜During the skills portion of the exam, they will require that you act out / talk about what you think is happening and what the next steps are. During the Code Blue simulations, they typically want the team to be doing the chest compressions, placing the leads, reading the rhythm, and performing interventions. I am sure you rocked the Mega Code portion ☺️
