Pump handle/bucket handle dissociation: This exercise assesses and trains our ability to expand our thorax in a front-back direction in the upper rib cage. In order to perform this exercise properly, it is assumed that we are able to take a solid abdominally-oriented inhale, as well as experience and maintain the contractions of the muscles involved in an abdominal brace. The practice is commenced by inhaling low and wide into the abdomen. After this initial inhale, an exhale is pressurized creating a gentle abdominal brace. During the subsequent inhale, the muscle contractions of the abdominal brace are maintained, while we attempt to expand the chest and upper thoracic spine in a front-back direction. This activity separates the pump handle movement of the upper ribs from the bucket handle movement of the lower ribs.
This exercise can also be done with a 4-7-8 count (inhale-retention post inhale-exhale). In this case, the 4-second inhale and 7-second retention are performed only with the upper ribs, while the lower ribs are held downward. During the 8-second exhalation the muscle actions involved in the abdominal brace are enhanced. This process is then repeated for repetitions.

Super helpful. I am a victim of the belly breathing trend all over the internet which can be shallow like the upper chest breathing. Now trying to work on the rib movement learning proper deep breathing. Thanks for the amazing explanation and technique

Thanks a lot Dr. Chris. I think we should just describe it as lateral and superior, but I REALLY APPRECIATE your description. I believe it's akin to saying, "it's just like a tilt-a-whirl bar when it comes up." When you think about it, it makes sense, but why come up with these metaphors?

Hi Von, Thanks for watching and commenting. In my experience, metaphors are vital for describing complex physiological topics to patients. Often things they need most are the things they can’t feel therefore they need to be able to visualize to even attempt to practice. The terms superior and lateral may have very little meaning to someone without an anatomy and physiology background. Also the movement of the ribs is superior in the upper thorax the feeling for the patient is often one of anterior to posterior expansion which may make superior and bit confusing.

Also if you could please explain role of Diaphragm as regard to pump handle and bucket handle movements as to its 3 attachments ? Thk you.

Hi Doc, at 1.48s You said pump handle movement happens at back as well, so does it mean flexion of thoracic spine ? What about its effects on lumbar spine ? Thank you
