Likely if we're to apply fictional and real science: The black hole is the cause of dark matter compression beyond comprehensive thoughts (and just the fuel overclocking in general), generating a black hole increasing in size, but also in compression and blast power. When the DMR structure can't take it anymore, the black hole implodes the superstructure and begins the process of hawking radiation (decay of an unsustainable black hole), of which once that happens, the massive blast pressure built up over the few minutes releases and detonates. Technically it's a bunch of magnetic and unrealistic practices I just made up but could make sense.

The meltdown was not an accident, it was on purpose. Dr. Helene Kaiser did it because her application for Chief Energy Officer got denied. (The own QS admins confirmed this)

At the part 8:05, once the player turns their view over on the reactor stats board, you can see the radiation levels uncontrollably rising. Once it reaches 10,000 R, it malfunctions and gives out an "Err" ("Error") message on the screen. I like this nice little detail. I don't know why, but it's cool.

That’s intentional, as it was probably confused as to what the hell was going on inside the DMR, hence why it starts dipping into the negatives before giving an error code like, “yeah, no, I don’t know what’s going on, I’m done trying to figure out what’s going on.”

@@raymondhuang2459 Good point, but the song used in the phase 2 meltdown is around 4 minutes 30 long and the time estimation probably wouldn’t be that good in the 1980’s, so 4 or 5 minutes is a better estimate for how long you would have to evacuate.
