And if You can't drag and drop action icons to menubar this means you don't have toolbar there. to do this, right click on QMainwindow in object view pane, and click add tool bar. sometimes toolbar is not added by default to qmainwindows

Of course, after submit the question the answer come on the next google entry: specifications.freedesktop.org/icon-naming-spec/icon-naming-spec-latest.html Just type on the "Icon theme", the text in the Name col from the Standard Action Icons tab.
