We insulated where possible in the areas that had stud walls. We didn't insulate the double brick walls as it wasnt practical. Keep in mind it doesn't snow here. We are more concerned with keeping cool in summer so our houses are designed to be well ventilated with big eaves to keep the sun off our windows in the middle of the day.

This just seemed like a lot of extra work to adapt to 2 x 4 floor joists.

Yeah, same deal in germany, if you don't have a cellar underneath. Every time I see some US house building stuff, I have the feeling, that they don't build to last. Good insulation + concrete foundation above it would be my choice. Also, with these wooden floor boards, you cannot use tiles for flooring. Even the elastic tile 'glue' will give up over time.

soil bearing capacity influences the AREA of the foundation (footing) (weight per area} not necessarily the depth - as well as obviously the imposed load? The depth of the foundation (footing) is determined by the strength of the material and presence or not of reinforcement and by its position in the soil relative to the depth of frost and or stable soil or bed rock? Typical minimum soil bearing is 2000 psi, meaning a 12x12" footing could bear (12x12" x 2000# or 288,000 lbs)? Assuming the footing remains intact (reinforcement). Continuous verses individual footings respond to potential of lateral as well as earthquake loads and can be dictated by local codes. Commentators, please make corrections.
