Fentanyl addiction actually destroyed my life for over 6 years. I suffered severe depression and mental disorder. Not until my wife recommended me to psilocybin mushrooms treatment. MAT saved my life! My doctor came to the gutter of PATERSON NJ where I was sleeping in a tent and got me! I was 100% homeless and was there for almost a year! I have to say doctor, youre doing a great job getting out this information!

I made the mistake of getting on fent, thinking I was taking the old school "blues" I kicked H back in the day with buphenorphine. This was a while different animal. I went through the precipitated withdrawal during induction, took more buph to try to rapidly push through (very bad move) best way I can describe it is being lit on fire then thrown into the artic ocean while still burning. It was easily one of the worst most painful moments in my life. That said, I literally crawled to my stash sweating and shaking and did as much fent as I could get in my body until it stopped. The "burneese" method is what finally worked. Cycling between fent and buph while very very slowly increasing buph while decreasing fent. It was still no walk in the park. Felt like a couple weeks with the flu...but I made it. After 2 years I'm almost fully tapered off from 32mg of buph. Own my own buisness. A loving family. And life is good. Stay away from that stuff...drugs aren't what they used to be.

I can never describe fent withdrawal except for billions of tiny little needles stabbing your skin and your brain and raising ur heart rate at the same time. Like I literally can’t explain it. Getting clean was awful. Took me forever to get there. I don’t know how anyone can get clean cold Turkey

My son uses fentanyl and so far won’t consider stopping due to his fear of withdrawal and being without and how to cope without it . He tried methadone for a short time. The “ system” Is failing and no one seems to care with enough compassion and understanding enough to figure this out . I’m scared every moment of the day

It’s because we have doctors who don’t wanna do the research. They have a one size fits all approach. It’s really sad. Every patient is different but doctors don’t get paid to cure anyone anymore it’s all about the visits.

The ONLY thing that has kept me off illicit opiates is methadone. Nothing else has even come close to working as well as the methadone treatment program I'm in rn. First time being sober for over a year since I was 17, I'm now 28.

I was on Fentanyl and Percocet for like 9 yrs. I was nearly but dead on yr 9 and my dr recommended I NOT go off when I went in to talk about backing off. I was diagnosed with Fibromyalgia. I called on the name Jesus and read articles on detox. Got with my family and within 9 weeks backed off all of it at home just using marijuana and family support. That was 2014. I am still delivered to this day without craving. If you want OUT too it’s possible. Oh yes! Call on His name. Jesus Christ All things are possible. My life is completely different to this day. What’s your opinion on methadone? You didn’t mention it when you were talking about MAT. I feel it saved my life, I tried suboxon just didn’t work for me.

You’re a great person Dr. B. I can’t imagine the hoops you have to jump thru with pharmacies, insurance, etc. Not to mention being a great Addiction Doctor i imagine is Very nuanced and challenging. Most Sub doctors are cash only and aren’t very nice, staff included. Stay blessed 🙏💞

Kratom was the only thing that worked for me.. not methodone not subs... I hit a year earlier in the mth.. it's possible just do the research and find your own path

My experience with the system is that they were not able to handle benzodiazepene withdrawal. Such hellish experiences. Drs would tell me benzos don't require titration... I wish we had 200 million mds just like you

I had the opposite, but I had a good dr at the time. I was abusing rc benzos, pyrazolam. Very weak stuff. So he taught me how to titrate, etc with it. Said if I ran out before hand, he'd prescribe me something. Got off it it np. I am have the exact situation right now with a subuxone clinic. They don't know what to do after 2mg...

I was very successful getting off of VERY HIGH DOSES of Xanax with phenobarbital. I don't hear many people mention how useful it is for benzo withdrawal but it really worked for me. What was also very important was that I took tons of vitamins, stayed hydrated got plenty of sunlight and continued to work and participate in life even though I was dealing with some suffering. In my opinion the worst thing a person can do during withdrawals is stay home in bed.

Bzd w.d. is the worst, life threatening pain I've ever survived. I often needed help from "outside services" due to all the restrictions on controlled substances. As for suboxone 2mg, just break it or use a pill cutter and try reducing .5mg every other day or so.

I started detoxing as of last Friday. Im now on day 6 of no more fentanyl use. It was the worst Suboxone put me into immediate withdrawal. Thank good Im past the worst part. I'm finally starting to feel normal. I just dont want to keep taking Suboxone. But I'll just follow the DR instructions.

Family doctor's can prescribe Suboxone, most professional and people in recovery have said it's harder to get off methadone then ANYTHING else also a lot of people at the methadone clinics are court ordered to be there and it's not a good scene. Fentanyl grabs and stays on the respirators a long time also there's the xylazine and benzos in everything! I know people are afraid of withdrawal but sometimes you have to be a little uncomfortable to get well. Having family support, M.A.T. team, 12 step programs and dealing with mental health issues etc are ALL needed. Some people I know use weed and or CBG as well but that's a personal choice and hopefully discussed with said people above. Because weed is mind altering it's not for me, also not drinking and I continue to pray in Jesus name and pray for others that go back out or are still in the madness. Recovery is possible and so worth it.

Coming off of fentanyl was the hardest thing I ever had to do. My kids needed a dad and they was worth me getting clean. I started methadone for a year and switched to vivitrol. You just have to find a reason to quit and choose a method that works for you. I'm 3 years clean plus my kids have their dad back.

Good job. I was on subs for 16 years. Raised a family, paid off my house and stayed the hell out of trouble. I tapered off last year and now I'm free. 4 more years and I can retire with a pension and a 401k. You'll never be sorry you did this.

ASAM PP synergistic bupe and KET infused slowly. I am just beginning the LADC path and doing my psychology degree capstone on OUD and Ketamine as a part of treatment. Lost my only child and stepped up to help. His first Opioid detox they used 16mg 😢

Dr. B. the O.G! 16 mg made things horrible for me. years i went from h, to methadon, to h again, to kratom. I went from kratom to subuxone, and they started me at 16. My teeth started grinding away the first year. I titrated myself, this was a year ago. I am now 1mg. a day, instead of 8mg 2x a day. Thanks to your videos. I might get to 0 next year (been my goal, to try to get to 0 by Jan-April). Also, these clinics have 0 idea how to get me off subuxone. They were like "at 2mg, you'll have 0 WD, there's no dose smaller". They didn't offer strips here. Why do I have to do these things against "clinic guidelines", like lying about my dose, so i can horde them a year later, and taper at home, in a slow manner after 2mg?

I've been on opioids daily since 2006 when I started oxys/heroin but I'm currently clean from everything except methadone and cannabis and have been for 4 years. Methadone works if you truly want to stop the illegal street opioids. I have a great life now have a full time job and my own place all of these things were impossible to attain until I started MMT (methadone maintenance treatment) 4 years ago.

In the uk some doctors can prescribe short acting opioids for detoxing - e.g I never successfully detoxed using buprenorphine or methadone. I was only able to quit because the doctor transferred me from 100mg methadone to 700mg Zomorph (XR oral morphine) per day and tapered this to 0mg over a few months. The withdrawals from morphine were shorter and therefore more manageable than methadone (10X stronger than morphine). This was in 2020 and haven't relapsed. Methadone was effective for opioid maintenance but I never managed to detox using methadone - I had been on methadone/buprenorphine for 2 years with not much success before we tried using morphine.

The issue now is fentanyl analogues stay in one’s system much longer. I’ve gone 5-6 days before even beginning to withdrawal. It’s crazy. Starting subs is so hard now. It took me 12 days of hell before I felt even some semblance of normal.

I was into heroin in the early 2000s & got clean right before heroin was replaced by fentanyl, both of them are bad but fentanyl is so much worse in regards to the effects it has on people who abuse it

Here in UK, day 1 we get 4mg in morning next day 8 mg morning, third day 16 mg..I know ppl on 32mg here. But now with nitrazenes & fentanyl detox / transition times are longer and withdrawal more intense. Best of luck out there ppl x

I’ve been sober from fentanyl for over a month a my former Suboxone doctor just deducted from 16mg to 4mg daily. He gave me last script and said “your going to relapse again, Suboxone is not enough” hinting the sublocade shot. What an awful doctor he is.

The issue is the half life. If you are dosing with sub every day it's never leaving your body. With a mean average half life of 38 hours. It only builds up. Shouldn't taper involve not only a reduction in milligrams, but a larger time interval as well?

16 mg is not enough for a lot of people. I don’t understand how practitioners have made that the norm. Especially for FENTANYL. 32 mg was the upwards dose that was used a lot a few years ago in the literature I’ve seen. It’s very difficult to find any practitioners that will go above that now. “16 mg covers most of the receptors” ive heard. I don’t know if there is any science backing this idea? Don’t understand what created the hype of lowering the dosage. Stigma?

One of the problems is that Fentanyl is that strong you can override buprenorphine with Fentanyl. I don't know why because I don't know alot about it but I can override 16mg of subutex with only 150mg of oxycodone. For some reason I don't believe bupe is as strong as doctors say it is and also I don't think buprenorphine is as sticky as as pharmas say it it. To me buprenorphine is quite easy to kick off the receptors with Oxy or Fentanyl. You don't seem to get quite as high with Bupe in your system but you can still get your opiates to work if you take enough.

Haven't watched yet, but a radical treatment that'd actually work would be funding clinics where users can be supplied clean needles and pure substances via a nurse.

Interesting idea. In the big picture it would be safer for so many and the facilities could offer mental health services and maybe insurance companies band the government would support and encourage it. It would save so many lives and have a huge impact on so many and society in general. It’s so hard to get the proper medical care even for basic health care let alone for complex addiction care.

The only other synthetic opiod that comes close to the severity of fentynyl withdrawls is methadone.

Methadone withdrawal was worse than the heroin withdrawal for me. I didn't make it. I roughed out the 3 days on klonipin and went on suboxone. It's now 4 years and I'm terrified of the sub withdrawals coming. So I'm just hovering at 8 MG and waiting for something so I don't have to go inpatient again.

Has the substance abuse community ever considered tapering fentanyl patches as a means of treatment? Or is there too much risk a person would abuse the patches? I ask because a pain patient that had the flat patches not the gel between 2 layers patches it was found tapering those patches down a lot more tolerable than tapering any other pain medication out there. The patient literally took themselves off completely by reducing them over time and then slowly trimming a little off the flat patches until completely off it. IT HAD TO BE THE FLAT ONES BECAUSE THE OTHER ONES WITH GEL WOULD RELEASE TOO MUCH AT ONCE! I say that just to be sure folks understand because the patches vary and some can give a life threatening dose if you trim them. Drugs like methadone and supposedly suboxone get really deep in your system and take a long time to get out. Methadone is a better choice than street drugs for sure but there are other methods if a person can really commit to quitting. Personally I wish more substance abuse specialists would start pushing for ibogaine treatments and make attempts at making it more affordable.

You're gonna think this is stupid, but why don't doctors sedate patients for the first so many days to get them over the worst of it? The biggest obstacle to addicts going in for treatment is the intense withdrawl. We all know that. There's got to be a way to help them sleep through the very worst of it, and then introduce Suboxone

Im in nyc and a friend mentioned that he went to a local detox center to get off opioids and they were very accommodating w Rx, free uber, medicaid etc..is that real? If so he said he never knew and always went to the ER if I had w.d. issues.. does anyone have experience? Tyvm

My methadone clinic decreased my dose 10 mg a month for dirty UA. Im on 10 mgs now,and I am doing amphetamines with fentynol. I had accumulated 4 years clean when I got on methadone program ,I relapsed on ice,and I am being shamed by the nurse and counselor whenever I reach out for help. I am being told everyone stops using once they get punished with the 10 mg decrease. So what can I do ? Where do I go now?

I’m tryna get off subs n I been leaning off using vikes n tramadol I wanted to know if I’m doing this right n if I asked a dr for help would I be denied my medication at a psychiatrist ? Because of struggling with opiate abuse ? Idk if I should even be getting off them but I never had them prescribed in New York it’s hard to find a good dr

Dr. B can you please make video about long relise morphine we use in europe and Canada called Kadian/Substitol. Not alot of info online about this meintrance drug. Do you thinknits better than methadone?

This video came out for me at a very opportune time. I have a question. What is done to assist a patient who seeks treatment but would be unable to start suboxone because it is too soon since theyre last dose of opioids? My issue is that I would have to wait 2-3 days (i think?) to start bupernorphine. I am already in heavy withdrawals just 12 hours since my last dose of my d.o.c. Is anything prescribed until bupernorphine can be started? Or is the wait not actually 2-3 days to avoid precipitated withdrawal?

The start of withdrawal needs to be initiated by doctor. Look, if in a bind look up clinical opiate withdrawal scale . Better than using. Crazy system . Good lick

@@DrBAddictionRecovery *gives Dr. B a good lick....slurp!* lol, had to call you out on the typo :) I've always gone with how I feel, and had no issues. But that was when I was dosing myself. Best bet is to look up what he said, the COWS thing. Everytime Dr's would tell me, "you need to wait 48 hours, you're on kratom/methadone". I wait till I start feeling medium withdrawal. Never sent me into precipitated WD. But the person knows their own body and when it's time best. like 9-12 hours at most. And was fine. Worse was when I was in detox, and they forced me to wait 72 hours and i was in pure misery. This is all anectodal of course, if you're unsure, COWs is the best source!

As much vitamin c as you can take a day and a bunch of immodium a day will hel alot with withdrawal. Which is the worst part

When Suboxone was a new drug i believe, i went to a great addiction specialist doctor MD and PsychMD, he told me the "sweet" spot would be 24-32mg. 32mg being the limit as it would bottom out due to its nalaxones action. Granted i was being treated for pharmacuetical drug detox including morphine, hydrocodone, oxycodone. A poly opiate user.bfyi. thanks for the info Dr B. I love your channel. PS I was taking the 8mg sublingual tablets which bioavailability might differ with these tablets compared to the 4mg strips possibly. I am now on methadone maintenance and clean on that.

Great video Doctor you are greatly appreciated❤🙏🏾💪🏾!!! I believe the system needs to change the practice of making patients suffer i know personally myself and others avoid treatment do to the fear of detox symptoms being unbearable.. Especially when there are treatments that paid insurance and treatment facilities in other countries use that make it alot more comfortable and bearable.. That and i wish there was a system inplace where you can commit yourself to a program that you have to complete and cant leave the facility till your done. I want and need treatment the mental grip the substances have over me are so powerful!! Again thank you for caring and understanding the importance of evolving treatment practices..

I wish Doctor B would do a video on the dangers of taking Methadone and SSRI. Those two drugs mixed together can cause sudden death in patients.

Dr b I have osteo genesis imperfecta and I break bones all the time. I roll over in bed and sometimes break. I’m not on fentynal but I am on a low dose of oxycodone when I break and In Oregon they will only prescribe so much. Im in massive pain life has become crap because I’m in too much pain to move. What would you recommend someone like me to do? When I was younger they were able to treat my pain and keep me pushing through to keep my muscles good and my body healthy but since people abuse the medication I can’t no longer get a breakthrough for when the pain is bad. I don’t need methadone or anything that’s gonna harm me more.

​@DrBAddictionRecovery Suboxone doesn't treat pain and suggesting a patient with legitimate REAL pain take A medication indicated for OUD/SUD IS HARMFUL and THEY WOULD HAVE TO ADMIT TO SUBSTANCE ABUSE IN ORDER TO BE PUT ON IT. DRS LIKE YOU ARE HARMFUL AND HARMING LEGITIMATE REAL PAIN PATIENTS. NOT EVERYONE ON PAIN MGMT IS AN ADDICT YOU SHOULD EDUCATE YOURSELF

I don’t have a problem with meds and I never have in the 44 years I been alive. I just wanna be out of pain when I break and need rehab so I can function and not have my body fall apart and die…but it’s like these doctors don’t understand my condition. I refuse to admit a abuse problem when I’ve always been able to taper off but when I break it hurts. I can’t push through therapy because of the pain. I’m kinda shocked this was his answer I’m not a addict and I refuse to be put on methadone. Just shows me he didn’t even read my message. Like every doctor out there they don’t listen to the pain problem it’s oh your a addict. I’ve been off and on pain meds my whole life never abused them and never been out. Why would I need suboxone? The problem is medical doctors and how there hands are tied from treating people who break 3-4 bones on small fall.

I was honestly looking forward to your answer but I am highly let down to be honest, I’m trying to get help when I fall and I break 3-4 bones and I have to have major surgery and have rehab but it hurts so much. I’ve broken 300 bones in my life and my body is very stiff and the doctors hands are tied due to doctors who don’t understand anything unless I go to a methadone clinic and admit I have a drug problem when I don’t. I only take 10mg oxycodone when I break but they can’t give people what they use too. But they can shove methadone down people’s throats and make them addicted to something far worse than oxycodone to the point there like zombies. I don’t wanna be a zombie I would like doctors to understand peoples pain and the right for a person to not be in pain.

That's why I hated Suboxone therapy and switched to methadone. It's more stable cause you don't have to worry about that. It's a full agonist

God bless you for making these videos!! You are by far the most helpful creator/medical professional that I’ve come across online! I love how you put a huge focus on education about different aspects about addiction. One quick question though, can you please, in the future, make a video on tapering off fentanyl/street fentanyl?? My husband and I need help. We’ve been trying to use less and less but we’ve only had one day sober in the last 3 years. We really want to get clean but it seems like there’s not much help for us and we’ve fallen into such a codependent drug addled relationship… other than the drugs we are a healthy couple and want to get better. I had 3 years clean before I met him and he had 8 years clean before he started hanging out with his now dead best friend, a month before I met him. Also, he’s on methadone and I stopped methadone because it didn’t help cravings at all and only made me gain weight.

Man I wish more doctors were like you. I’ve been stuck on an ineffective dose of methadone for several months and even though I’ve told my doctor many times that I still have cravings AND even withdrawals (esp at night) he just will not listen to me… to the point where I’ve had no other choice but to self medicate with high doses of loperamide on the side just to keep from feeling sick all the time. It feels so damn stupid to be on MAT for so long with an ineffective dose! 🤦🏻‍♂️ From 🇸🇪 btw, and I’m on 80 mg.

80 mg is a pretty sizable dose and I’m sure there’s a lot of factors that go into the right dose for a person. I’ve heard of some folks on some much higher doses but they are the rare ones. Have you ever asked to have your peak and trough levels tested? Some individuals metabolize the medicine quicker than others. IMO if it’s been months at that dose it should have calmed down already so perhaps you’re one of those that needs split dosing. Doesn’t hurt to inquire.

@@lh98 That’s exactly it though - it’s just not, for me. That’s what I wish the people involved in treatment would have a better understanding of, the fact that things like tolerance, metabolism and other factors are VERY individual. Before I got into treatment I used to mostly chew fentanyl patches and I used at least one 100 mcg/h (11 mg total) per day, if not that then I’d need around 2 grams of oxy or heroin and in the end I started feeling like not even that did anything for me anymore. Yes, while I was actively abusing I was obviously looking to get high, but now I only want to not have to feel any crawings or withdrawal symtoms. Basically, just wanna feel normal. I don’t though, because I still wake up way too early in the morning (at like 4) and feel withdrawal starting with these nasty fkd up temperature swings and cold sweats. I’m not saying that I’m going into full withdrawal or that my situation is completely unbearable - ofc it’s gotten better as they have increased my dose up to this point. It’s just… ”not unbearable” is not going to be enough to keep me away from eventually just using again - I know that, just as well as I know that I’m indeed completely fine on a higher dose. I only have to pickup my doses on mon, wed, fri and ofc I have tested taking more to see what difference it would make, and it does make all the difference I was hoping for… which is why I’m so frustrated. It doesn’t make any sense to keep anyone with any type of disease on an ineffective dose of their meds. Anyway, I’m also a nurse myself btw and really didn’t mean to ramble… I’m just in a very complex mess at the moment and this video kind of brought up all the things I wish every doctor in this field were more aware of. Edit: Oh and to actually answer your questions, no I haven’t had any tests like that done yet but I’ve recently requested to switch doctors and my new one mentioned doing that and he at least seems a bit more interested in actually helping me and meeting me where I’m at right now.

There is nothing wrong with Jesus or Suboxone, if each plays the role it needs to in a person’s life. In terms of habits, there is nothing wrong with the term, intrinsic sense. In fact many habits are great. Exercise, fruits and veggies, washing your hands and indeed subs. They can all save your life. What is important is that each habit is approached with a healthy sober Mind to give context and meaning in one’s life. Otherwise, they can all corrode and kill. Be it subs or Jesus.

Dr B. I have done 4 shots of Sublocade and became VERY ill while on it, and dealt with nothing but issues. I elected to come off of it on my own. Is this advisable? I am closing in on two months without the shot, and am feeling perfectly fine, in fact, better than I had felt in the months prior. I appreciate your input! -Josh G

You have to wait 2 or 3 days before get subuxone, not if you still using fentanyl or any opiods but yes you can after you wait couples days whit the whitdraws.

Indeed to get suboxone, I am not allowed to use benzos for MS related seizures. My history with benzos is actually very good, I under took the amount I was allowed to take, but now they yanked it and my blood pressure went nuts and I went into status epilepticus

I'm on 12 16 is alot to come down from, taper can be small incremental decreases over time,can be tough but WORTH IT Suboxone saves lives,Harm reduction saves lives😊
