Basically the story behind this song is that the parents of the children weren't around and it was only the big sister and the little brother that had a mental disease. For obvious reasons the sister couldn't pay for his medicine so she decided to sleep with men for money, ending up dead by one of her clients while the brother was watching through the keyhole of the bedroom.
